Could Rectogesic be making pain worse??

Hi all

Anybody experienced MORE pain and soreness with using Rectogesic? I have been using it for 18 days after GP suspected fissure (but none felt/seen). I'm waiting on specialist appointment in February but meanwhile she prescribed GTN.

The headaches went after a few days so I thought great now it can heal me up. But I've been sore for 18 days straight - normally I get some respite a few days/week soreness after a BM then I get better, until usually a harder BM sets me off again. But now it's constant soreness which is worse after a BM.

I am sensitive to medicines - could it be I'm allergic to the cream? Anyone heard of this? I know headaches are main issue but leaflet doesnt say much about skin reacting and feeling MORE sore. I'm tempted to stop for say a week to see if I get better.

Any ideas be amazing...

It's quite possible that the ointment is making things sore. One way to tell would be to stop it for a couple of days, and see if the pain improves. Or try using it fewer times per day instead. Do you use it before a BM as well? Perhaps that's increasing soreness afterwards. The anal area is very sensitive and can react painfully to foreign substances. I would also suggest only applying it externally (wipe off any excess to prevent a rash) if you can't handle internal application.
8/16-12/16: Fissure due to antibiotics
5/17: Botox to sphincter, fissure healed
9/19: Trigger point injections and pudendal nerve block
11/19: Botox to pelvic floor
8/20: Botox to pelvic floor in new location.
On and off in pelvic physical therapy

Hi

Thanks for the advice! I have stopped using it today. It's too early to tell but it isn't as sore today, but I need a few days to see if the Rectogesic was actually making things worse. If I do restart I'm going to do what you suggest - external only and maybe once a day not twice. As for if I use before a BM - it varies sometimes yes sometimes after. Thanks again. I'll see what happens.

I had skin irritation from it and it kept giving me a yeast infection in my anus and vaginal pain/yeast infection there too. I know that sounds weird. I only did a small dab at bedtime only and that lessened the issues. I did the cram for 7 weeks. Continued to have pain until 9 weeks when I took a pill for yeast infections. Pain went away. I do believe that it helped heal me though.

Okaybum - love the name! That's really interesting as I'm beginning to think it's just me as everything online says it's so wonderful etc. (except the headaches which stopped for me anyway).
I've stopped using it but only for one and half days so far. Yesterday my pain lessened. I am going to give it a week off then maybe do what you did and small dab externally.
So are you healed entirely now?

Be sure to wear finger protectors to lessen absorption. The headache was really the least issue. I'd get diarrhea the first day but then very slight nausea that lasted the full 7 weeks. Using a finger protector and a small dot at night made that much better.

Doctor said I was healed, she couldn't see the fissure. I'm at 13 weeks of healing from a fissure that last retore on 20th September (I'd had it since May)

I have no pain, occasional sharp BM depending on what I eat (it's 99.9% soft but I ate too much popcorn last week) but it doesn't bother the fissure site. Every now and then I will feel a little twinge, but I'm doing ok. I don't feel the need to guard it during a BM.

I'm still on a boat load of stool softeners and using nappy rash cream occasionally to ensure it doesn't dry out which being dry is what I think caused my issues.

But I definitely think the GTN helped but I did have a skin reaction to it. Everything down there was sore. I was able to prove that as I stopped it for 2 weeks, got better, but had fissure issues so restarted it, discomfort immediately returned. It went away upon stopping it again. Definitely not just you

I've been off it for roughly 4-5 weeks now I think, I'm doing really well. But I'll be honest, it took 5 weeks on the stuff before I could tell it was working.

Hi

Since May is about how long I've been having trouble - although I had this same pain back in 2014 for months - it feels like something sore on the left hand side mostly. When it was last inspected colonoscopy he banded a hem but couldn't see a fissure. He did a stretch (yuk) but to be honest nothing specialist did helped it just got better after several months. This time I am now booked in to see specialist on 7th Feb and hoping he/she can actually see something which is causing this pain as my GP said it 'sounds' like a fissure but couldn't see anything from an exam. SO I don't know if the Rectogesic is even treating a fissure or not.

Can I ask you what kind of pain/symptoms did you have from a fissure? I have little pain until after BM- the BM itself doesn't hurt, no sharp pain and there is no blood ever. But once I've done the BM then the aching,soreness comes on and stays all day. Usually it just hurts on left in one place about an inch or so in but sometimes whole anal area and even into buttock. Does that sound like a fissure???!!

I have to keep stool soft and that is really tricky for me (and you it sounds like!). I've been using clingfilm for the Recto so that lessens the headache. I really don't know whether to restart the cream or not now since you reckoned it took over 5 weeks to work.

Hmm...to be honest, it doesn't to me but I'm not an expert and fissures can be different. I've had fissures throughout my life. They all start the same. A hard, large poo that that scrapes coming out. Queue the blood in the toilet. Now every time except when I was 16 and then again last year and this year, those fissures healed up without doing anything.

Last year, I had 2 internal fissures. As soon as I took a stool softener, they were fine. I had to take it for about 12 weeks but I was healed early on.

This year, I had a hard, large BM, and nothing happened. I proceeded to have 6 more hard BMS that fateful day and out if dryness or something else, by the end I felt a slight, sharp pain and a pop of my skin coming apart. There was no blood. However, the soreness repeated. Now I tried to get my BM soft. I did stupid things that week though and things ended up super sore and bloody.

Funny thing is, two doctors have looked at it and said it's too small to be classified as a fissure.

Main differences between this fissure and the others last year: just soft isn't good enough. I had to have diarrhea BM for about 2.5 months, this required a boatload of stool softeners. I had to buy a new toilet seat as our other was too big and the pressure caused that dumb thing to almost pop open.
I've never had the pop open feeling before. It was on the outer part so it also felt like a papercut and would sting after BM.

I'm sure it's healed now at almost 14 weeks. I've relaxed my diet, I'm ok. Still on 3 movicol and two doses of docusate sodium but I've slowly started to reduce. This reduction is going to take many more months and I think I'll stay on a single dose of movicol throughout Aussie winter as it always seems to hit me at dry, winter.

Now, the only time I get pain around my anus was when I wasn't relaxing during a BM or if I ate spicy or citrus foods.
I recommend breathing in and out, deep breaths before and during a BM. It was recommended to me and helps to relax your muscles to allow a BM. That may help you.

Hi

Thanks for all the info- I'm sorry to hear you've had this a long time. I will try the breathing - this may help if my problem is caused by tight pelvic floor.
I agree my thing doesn't fit a fissure as such- for starters I have never had bleeding and I've had this issue on and off for five years. And no real sharp pain with BM ever, it starts after the BM and builds. I'm waiting on my colorectal appt in Feb then see what they say and if they can't find anything then I will look down the pelvic floor therapy route. I guess everyones pain is individual so what I feel could be different to you etc. I just want it looked at now even though that's not much fun!!
