Some people use cotton, some use gauze (I found cotton to be more comfortable than gauze). According to my CRS, the cotton is used to absorb any blood/discharge; the alternative being lots of underwear changes. It also helps to keep blood/discharge off the incision site/sites.
As far as how to apply cotton, I just place it between the cheeks; no need to go any further. It can be a little uncomfortable at first, but I found that stretching the cotton a little made it more comfortable. Over the last month, I have got so used to it being there that I don't even realize that it's there, until it's not. Immediately after surgery, I would change them every hour or so and clean the area with warm water. Now, I change them as needed (3-4 X per day).
