34 YO Female Newly Diagnosed and in AGONY

Hi there,
I've been suffering with hemorrhoids for 11 years following being bedridden due to a head-on car accident in 2001. The first time I had any pain was in the hospital, consitpated severly, from the multitude of narcotic pain meds. Since that time I have gone to several doctors who dismissed me as having "regular woman type hemorrhoids". Finally last Thursday I was able to secure an appointment with a Colorectal Specialist after talking my new OBGYN into the referal. I have several Grade IV internal prolapsed hemorrhoids with chronic (scar-tissue evidenced) healed fissures and a large open fissure right now.
I AM IN AGONY.
The surgeon recommended total hemorrhoidectomy to remove the internal and externals, but I am terrified as she told me I would be pretty much bedridden for a week following and in horrific pain for at least 72 hours following the surgery. Prior to that happening, I have to have the fissure healed.
She prescribed me Konsyl (a high dose psyllium fiber supplement) and Diazepam cream 3 times daily, sitz baths (or regular hot baths as many times a day as I can) and Vicodin for the pain. NONE OF ITS WORKING.
Unfortunately, I started the Konsyl this weekend and had a weekend full of outings with my family so I wasn't cognisent of the added water intake I would need, (thus it produced some acute constipation but with urgency... a double whammy in the anal-fissure world I am discovering). SO this morning, I skipped the Konsyl and opted for a double dose of Ducosate (stool softeners) a gallon of water and the diazepam cream. I'm still wanting to scream all day here at work. I've had 17 broken bones at one time, (both legs, a foot crushed, an arm crushed and other internal injuries, and I'm pretty sure that this tops it all).
Does anyone have experience with the Valium? I'm about to call my GP to see if I can get a small trial prescription since having a relatively unsuccesful BM this morning I've been in terrible pains.
Has anyone else had experience with both high grade Hemmy's and AF's???
Please help me!? PLEASE?!

Hi, gosh you've been through a lot. I don't know where to start. My hemmie surgery is what started me on the path to anal fissures and now stenosis. First when you're trying to heal a fissure you don't want to take a bulking type laxative, it makes your bm too large. Try miralax or milk of magnesia. As far as your hemmies go if you decide to have the surgery make sure you have an excellent surgeon who will explain the procedure and any complications (scar tissue is a biggy). Also Valium can help a little with the spasms but you shouldn't take it on a regular basis and Vicodin is constipating . Sitz baths are great but I found sitting on a heating pad works pretty well too. I never found stool softeners worked for me but if they help you it's great. Try not to use dry toilet paper, it only irritates the area, wet wipe are good. Gee I don't know what else to say. I hope you find some relief soon. I also had my hemmies for 11 years so I know how bad they can make you feel. Take care of yourself Suzy.

I've been on a strict wet-wipe regime for about 6 years now. I'm thinking that this Konsyl (the fiber) is what re-injured me. I was actually feeling like it was healing this past weekend. The stage IV prolapsing and the surgery to "remove" them terrifies me... especially all of the horror stories you hear. Very few "happy ones". But now it's all I can think about. THis pain is all consuming... I really thought, "it can't be that bad, I must be a woosie" but seriously when all you can think about is how much pain you are in, it's pretty hard to do anything else productive. I think I am going to leave work a little early today and go home and sit in a hot bath... called my CRS to see if she can do anything for me for the pain... IB Prophen is my mainstay during the day.. I know Vicodin is constapating so I avoid it at all costs.

Hi, I can understand. My hemmie surgery was an emergency. I was bleeding profusely and had already lost about 1/3 of my blood. They couldn't stop it so I had to have transfusions and then the surgery. Back then they kept me in the hospital on pain meds for about a week so that actually helped but now they send you home the same day. I was told that scar tissue could cause a narrowing of the anus but it happened so slowly I never put it together until I actually tore and even the first time it was no big deal it was the second time and the fact that it wouldn't heal that finally sent me to the doctor. If you have the surgery work out your pain management before you have it done. Try to get your bowels moving as soon after as possible because that really helps and don't get constipated. I don't understand why they want you to heal the fissure first though. They are going to stretch the area to remove the hemmies and I would think that might cause the fissure to retear, but I'm not a doctor. I had my hemmie surgery 30 years ago and back then they used morophine and later demerol for pain management. I don't know what they use today though. I hope your doctor can give you something that will help. All the best Suzy

Hello,
This board is a nice help, I have looked around and learned allot. The diet tips is a good place to start also. To then customize what works for You.
I also to help get thru the healing, I use an old saying "Believe in the Wisdom of the Body to Heal Itself" also I have a tiny ticking clock by my bed, I listen to the tick, its a slow calming tick tock, and tell myself with each passing moment I am that much closer to healing.
I find some positive mindful messages will help. As the day can seem long.
*** Suzy you are similiar to myself, as I had hemorrhoid surgery and not can get a fissure. I eat well, and correct but it still can happen.
I Wish you all healing and most of all Calm.

Thanks so much everyone,
I went to my General Practitioner yesterday as an emergent case. I was in so much pain from the spasms (which were completely my own fault after upping my fiber very abruptly and not getting enough water, causing my morning BM to be horrific) and he put me on Flexeryl, a muscle relaxer. Then I went home took one, and soaked in a hot bath for about an hour and could function. The effects wore off within 2 hours so I waited it out for another 1.5 hours and took another flexeryl and another hot bath. The good news is the flexeryl and the Ditazepam cream knocked me out. My CRS office called me back finally around 4:30 and said she would prescribe me lidocane cream as well to help with the pain when I have a bad flare. Not sure the rationale with that since the pain was all internal, but willing to try anything. Yesterday if they could have just anestitized me and done the surgery, I would have gone right for it. NO questions asked. I did get enough respite to sleep all night and today the spams seem to be well tollerated. I have yet to have another BM though (I did have one yesterday afternoon after taking the first flexeryl because I was certain that the morning one was "cut short" due to the pain.
Now I am avoiding solids pretty much all together. No real appetite anyways. Hey silver lining is the 10 pounds I wanted to lose shouldn't be an issue.

KPeff78, sorry that you are having such a bad time this days, I can not stop imagining how much pain you had suffered during your accident and now this.
let me thank you, yes let me thank you, because today I had my fissure reopened and burning like crazy after weeks of manageable and some time pain free bliss and reading your post just show me that is others that are suffering just the same and more and that I am not alone.
Selfish me, I am so sorry for your hard time.
And yes I read of another member having all done at the same time, from fissure repair to hemorrhoid removal in and out and he end up in the emergency room with such much pain that they have to sedate him after the surgery.
So take it easy and take one step at the time to resolve your afflictions.
One more thing, something that help me with the bms and keeps them smooth is coconut oil by mouth two spoonfuls a day.
My fissures are the result of diarrhea no constipation so that is why I burn every time I have to deal with lose stools.
