AGA Guidelines for Anal Fissures

Here is the American Gastroenterological Association (AGA) guidelines regarding the diagnosis and care of patients with anal fissures, which may be of interest to many of you:

It covers a wide variety of topics ranging from diagnosis to treatment, including conservative treatment, topical nitrates, LIS, anal dilation, self dilation, Botox, etc. It also covers the success rates of many of these treatments.

Nice information, I try to absolve all of it, but it kind of made my head spin with so much contradiction in conventional treatments.
I guess everything have its side effects and different rates of success.

The statistic that surprised me the most was the high success rate of Botox injections (as high as 73% in one study). If I'd known the success rate of Botox was that high I might have tried it before opting for LIS.

I know, I was in the believe that botox was not a good alternative, I think now that I am done with the nitro (it didn't work) and diarrhea control (most of the time) I am going to ask my doctor about the botox infections and see what he say, he mention surgery but not anything else beside the creams.

Have you tried Nifedipine? The normal prescription is .2% but my CRS increased mine to .3% and it really helped. The compounding pharmacy I use mixes the Nifedipine with equal parts lanolin and vaseline, which is very soothing. I think it helps keep everything soft and protects the fissure from further irritation.

I believe that the nitro i used is the same equivalent to Nifedipine because is a mixture of both.
I was at 2%, since august so I know that is not working, I have appointment in February, I will ask if is anything else before surgery I can try, trust me, right now, I just reopened the darn thing today after almost a month feeling much better, I am feeling like screaming for surgery right now to end the discomfort. But I am so afraid of the results.

For me, the surgery (LIS) was a breeze. I went to lunch immediately following my surgery and enjoyed relaxing and watching tv for several days afterwards. I wouldn't hesitate to have it again if I needed it. I'm assuming you are going to a board-certified colorectal surgeon with lots of experience? My CRS said LIS is one of the easiest surgeries they do.

You are one of the lucky ones, I asked my doctor to recommend the best gastro with the best track record so I have a mature experience gastro surgeon, I am no afraid of the surgery as I have read what is all about, but what is bothering me is the fact that I have developed a large skin tag where i know for sure that the fissure is hiding and it needs to be remove, I wonder how bad will be to have stitches in such a very delicate area and how long will take to heal, not the LIS but the removal of such a large piece of skin.
