I have to agree with thisfissurecankissmybutt. Gosh, all the waiting you did to see this guy.
I've seen three top specialists for fissures, and while none of them feels surgery is an option for me because of other health problems I have, that is not the general consensus when it comes to surgery. Further, there is no reason not to try a topical like nifedipine, which every CRS I've ever talked to and heard of nowadays recommends at least giving a try. Botox is another option as well. Topical medications give you anywhere from a 50% to 65% increased chance of healing without surgery, so there is no reason not to start with that.
If you can do it, I too would suggest a second opinion, and while you're at it, since your insurance is running out, you might want to get an appointment with a third one as well. You wouldn't necessarily have to keep both appointments, but it's better to have them on the books in case you run into something similar to what you did today.
