Internal sphincter connected to autonomic nerve system, which means it cannot be controlled by our mind. That is I think whole challenge in healing as body sphincter with open wound reacts automatically to constrict, causing pain and slow healing.

Probably yoga or some non-traditional approaches can be used to relax this muscle apart from chemical, botox or LIS
