Welsh, I have a nice "pocket," called a rectocele. (Please forgive my correct American usage of quotation marks. I prefer the British grammarians on it.) Sometimes stool will sit in that poacket, and form a nasty #1, and once that has passed (and it's usually not too large) it gets better.
The wost for me is what appears to be a #4 on the scale, only it is not fluffy, it is firm and filled with visible fiber. I have decided to break up my once nightly miralax into smaller doses WITH my meals in hopes that it will help keep the fluid WITH the fiber.
Botox takes longer to kick in than the LIS, or o I've read. But it seems to me that the docf would want to look at those fissure sites at least once around the four week mark. My appt will be 4 weeks from surgery date...can't wait to ask if all my wounds good enough to get into ocean water!!! So ready for the beach!
How many grams of polyethylene glycol 3350 are in each movicol sachet? One Miralax is does is 17 grams.
I've been meaning to log my food in vs. visible out. I think my body is on a three day exit plan. That means I'm still technically constipated even when the stool is soft.
The things we get accustomed to that others would find very disturbing!!
Spinach, broccoli, and berry seeds are easy markers for knowing the bodily transit time. I'm thining knowing that could help me more easily determine how to increase/decrease my miralax doses.
My husband asked me last night, "You on your butt blog?" My kids thought it was a wise crack...pun intended.

"Butt Blog" waaaaah that's brilliant mom. Thank you for bringing that term to me. That needs to be used more. I'm going to rename all my updates to Welsh Butt blog!!

Movicol "Macrogol" (whenever I see quotation marks it reminds me of Dr Evil's "laser" scene) polyethylene glycol 3350 has 13.125g.
I can have 1-3 sachets (powders mixed in with 125ml water).  For long term use I can have 1-2 sachets a day. It kinda tastes like plastic water.
Is Miralax sachets of powder too or in liquid ready forms?
Found this on the Movicol website which was very interesting. Shows the digestive system working which I've never seen it before.

I've also just bought some prunes. These however live up to how I thought they would taste as a kid. Guh-ross. Having to chew them to a pulp also doesn't help.
Just had a vanilla lattй from Starbucks too. OMG coffee has never tasted so good.
I have no idea how long my food takes to go through. I'm hoping its the next day. Spinach, brocolli and berry seeds. Hmm. I'll try these in the next few days.
My raw coconut oil has just arrived too so I'll start using this today.

Miralax is 17 g of peg3350, but doesn't have the other ingredients that the movicol has added to help with preventing electrolyte imbalance.
Miralax is a powder/granulated substance to mix with any beverage, hot or cold. I can't taste it all.
I'd have #6's too if I took it 3x a day. I'm thinking 1-2 tsp with lunch and 2 tsp with dinner. That puts me just over or under the 17 g daily dose. Maybe I should aim for #3's.
I made a mixed berry smoothie from frozen berries with minimal yogurt and some almond milk. I know almond milk has more calcium...but it also has a touch of fiber that cows milk doesn't have. Added some vanilla. Boy was it a nice change.
Raw coconut oil is great, by mouth, or by topical application.

Hey Rav, I was told by my CRS that LIS was recommended for males more than females, he said it was because males have a longer sphincter and so the likelihood of incontinence is greatly reduced.
He barely examined me during my one and only appointment as I was very sore. He booked me in for surgery where he said he'd do an examination under anaesthetic and decide what's best for me. During surgery, He excised both of my fissures (removing the scar tissue that was unable to heal- changing them from chronic fissures to acute fissures) and gave me Botox injections.
By making my fissures fresh and acute, I have more chance to heal with creams and the Botox will help relax everything when I have a BM.
So effectively, as long as I keep my stools soft, the Botox should kick in soon and my stools will slip passed the fissures not causing them to retear.
Well- that's the plan anyway. If this doesn't work by my next appointment in September, then I guess the next step would be LIS.

Do you feel the botox has kicked in by now?
Any looser down there?
- Fissure developed in Jan '13
- Started rectogesic in Feb '13 and diltiazem Apr '13.
- Got botox Jun '13
- Healed by Okt '13, although I still had some irritation for about a year.
- New fissure April '22, healed June 2022
- New fissure 24. December 2023

It's been exactly 2 weeks since and I don't feel any different JHH, if anything, my last 3 BMs have been a bit larger than I'd like so I've felt them move passed the fissure. I'm not sure how its supposed to feel. I've been reluctant to use my sphincter muscles too much whilst having a BM because that's when it'll tear.
So to be honest, I don't think it has kicked in. Fingers crossed it'll be in the next week maybe?
Can anyone shed some light on what it should feel like. I did have a crawling feeling once or twice this week. But only after putting Diltiazam on.

Sorry bout that. I guess Im in the same boat, just 10 days ahead of you. My pain has changed, but I feel just as tight, so I dont know what to make of it...

actually I have gone through LIS that relieved me of pain n fissure aggravation. but today n few days earlier my one very minor fissure slightly disturbing bcz of disturbed bowel movement but still I m far far better than pre op. I just have very minor sensation of some wound but it starts healing v quickly n effectively

Been reading your thread with interest :) 
Firstly, did you try out the Raw Virgin Coconut Oil?? I found this wonderfully soothing & moisturising and it really helped that whole area generally "feel" better after a BM...
Secondly, I immediately zoomed in on the word "Chocolate" - LOL!....something I love, and started to increase as I felt better and better and was further inspired to eat more, when I discovered it's a source of Magnesium too!...If you're still refraining, I'd say don't be too scared to give it a go again. To my great surprise, after months of refraining, it did me no harm at all and my stools remain soft as. Coffee is OK too but absolutely all in moderation....
You sound like you are doing really well in your recovery and unfortunately, it is almost best to measure your recovery progress by the month than the week because it is slow. (But you get there, I hasten to add!)
Yes, it's completely normal to feel some discomfort and to "feel" the fissure(s) as stools pass over them because everything is so sensitive down there. Even though "healed", I still feel the odd stool pass over that area, which is now covered with scar tissure. Even with scar tissure, it is a delicate area but my CRS reassures me this is totally normal.
I can also still feel some minor discomfort these days PRIOR to a BM when the pressure of the stool in the rectum pushes down onto that area. This manifests itself as a minor twinging sensation but nothing major and actually when the stool passes, it goes, so again I know it's just the area reminding me to be careful and never to add any additional pressure by pushing or straining on the toilet and I just don't do it anymore! (even though old habits die hard and the temptation is there at times....)
The pressure issue you describe is an interesting one because I also experienced this during my recovery. It kind of drove me a bit potty because at this stage I had just stopped using Diltiazem cream and did not have any of that awful searing knife-like pain. For me and retrospectively, it was almost entering another stage of healing a chronic fissure....I had a few weeks, where particularly in the evenings, I'd experience this intense pressure in my bum and feel the need to pass wind, but even if I did, it did nothing to really relieve that feeling. I attributed this to continuing spasms in the area and it did actually pass after 3/4 weeks. It was just uncomfortable & annoying and I couldn't really figure it out to be honest!
My healing seemed to spiral (slowly!) once I'd passed this stage, so just thought I'd pass on hope :)
Good luck to you with your continuing healing and don't ever lose your sense of humour!!!
