You can stay on the nefedipine as long as you like, it is just a BP med that's been compounded into a cream, and as you know, many people take BP meds for their entire lives. I wouldn't worry about changing to diltiazem, as they are both very equivalent.
When I had an active fissure, I noticed a huge decline when I stopped using my nefedipine. So stay on it until you have been diagnosed as 100% healed.
Also, try to regulate a high fiber diet to ease your mind about diarrhea. If you try to eat pretty much the same things every day you will probably get a more consistent stool, but that can be a very difficult thing to achieve.

I have heard of using vick's vapor rub so I would think carmex would be the same. I can't use it though-too intense! Image

i don't necessarily think that using anything thing with a minty zing on a fissure would be soothing but i think everyone has a different level of tolerating intensity.
it may work.
i definitely have used vapor rub on skin tags and that brings a world of relief. but then again, a skin tag is a closed, self-contained abnormality of the anus, whereas the fissure is an open wound.
also, when using your other creams, not sure if there would be an interaction between whatever ingredients are in the carmex lip balm or vicks, etc. so be careful.

I used it 4-5 times a day. BUT, I had VERY strong sphincter and VERY strong and painful spasms. Since you do not have those, maybe you do not have to up the dose. You may try though and see if you feel any difference - it is all individual , really.
