I think my specialist is a bit odd !

ok,
so I have been using Nifedipine 0.2 with success for the last 2 years, I have done about 4 different courses for various lengths of time. Well every time I stop the treatment I can feel the fissure come back after about a month.
I went to this new specialist and he told me he does not like Nifedipine because he trusts Nitro better, he says the Nitro is more effective. The Nifedipine will take longer to heal but does the same thing, or so he thinks.
He puts me on this weird dosage of Nitro 0.125, he says this is better because it will not cause the bad headaches that 0.2 does. I have never heard of this anywhere online, and yes I have done much research on fissures ! I think this new specialist is insane ! He is a nice man though.
Well three weeks into his treatment I can now feel some stinging after bowel movements, When I was on the Nifedipine... I felt better with less pain. I am using this Nitro twice daily, the same amount I was using with the Nifedipine.
I made an appt to go back and see him to tell him about my results.
I think it is better to go back to my Nifedipine ! I feel that this dosage of nitro is not strong enough, not too mention I am nervous to use too much because of the headaches !
any thoughts ?
he also told me to put it on the tip of my finger and insert it all the way in (like the whole finger) he says most people use the ointments incorrectly. They do not put it in far enough to reach the muscle.
last question... would Diltazem be even better then Nifedipine ?

well i can't vouch for either of these creams but i do know that my CRS did not recommend putting my finger all the way in my butt to stretch it or to clean it or for anything....
he even wouldn't put an anoscope or his finger up my butt to look around because clearly there was a fissure there.

for each person there will be a response with these creams. sounds like you had a positive response with one over the other. i think for you, that is what you should do and continue to do if you continue to receive benefit from it.
sometimes these butt doctors are a hit or miss, just like with any other doctors.
but in the end, it is our decision (of course, with the help of these docs) but it is up to us to find that needle in the haystack....
and with these fissures, that needle is either microscopic or in a whole other haystack!!!!!

I used an applicator with my ointment which is narrower to insert than a finger. Just a suggestion.

I used both - Nitro 0.2 and later Nitro 0.4 and Nifed., at the end both alternatively during the day LOL.
I used them only outside and my CRS told me that creams are absorbed transdermaly anyway , like Nitro patch is absorbed for heart patients - that is why you get headaches although you put it on your anus only LOL !!! It ends up in the whole body , so I really think that place of application is irrelevant - but it is just me Image .
Best of luck and definitely don't force your finger or anything else inside if it is painful to do so.
His prescription for 0.123 or something like that is not based on any research and I would ask for 0.2 if I were you. Headache is normal side effect and nothing that Tylenol would not
fix.
I found Nitro more potent and faster in relaxing my muscle after application, but everybody is different.
Keep in mind though that long term use of those creams can cause you to become sensitive to them ( develop strong reactions of burning or allergies or pruritus ani ).

I definitely prefer the nifedipine. The nitro can be irritating.

Also try reading "Refernces" topic threads - ton of info about it all. You can use "search" engine here and type in any "word of interest" and will find any thread ever covering it !

I have used a lot of Rectogesic 4mg/g, and I used a syringe without a needle to get it in. It's small and easy to use. Getting my finger inside when my fissure was Image wasn't really an option.
Rectogesic does help with the spasm and pain for me, and I will use it again if necessary, but I didn't like the side effects much (headache, dizzyness, low blood pressure, increased heart rate).

Corsi, when you said on another thread that nitro gave you tummy upsets, were you joking?? I've been upping the nitro because my fissure is playing me up, but every night for the last 3 nights I've had a tummy upset -(as in having to poop LOTS of times!! Image ) I thought it was a bug the first night but I dont feel ill and i havent eaten anything strange, so i was just wondering if it could be the nitro Image

Well I think I am right, started back on the Nifedipine and my fissure and anal area feel much better and more relaxed !
going back to see this specialist next week, tell him what is happening !

I don't think I said that?? Or if I did, I couldn't have been sober... It hasn't affected my tummy, as I very seldom eat it (Tabby Image ).... I say a lot of stuff...find the thread and I owe you one!
