New Here - treatment questions - help!

Hello,

So I am new here with what my GP tells me is an anal fissure also had/have haemorrhoids on and off in the past. The GP hasn’t actually looked at my bum so the diagnosis has been based on symptoms described over the phone which are a terribly sharp pain and then scrapeing sensation on BM followed by feeling really sore and burning for a period of time after. Doesn’t always bleed - had a bad time just before Xmas when I had 2/3 days of bleeding but since then it’s just been the pain on popping and burning after.

The GP gave me Unirod HC and Rectogesic which worked the first time back in Dec but the issue is it seems to keep coming back!!

I used the Unirod for 3 weeks in Dec - tapering down from 3 times a day week 1, twice a day week 2 to once a day week 3. Then was off it for about 10-14 days before I had pain once more and had to use it again. This time used it for just a week. Then I managed 2 weeks off it again and then had to go back on it for a couple of days. Managed another week or so off it now been back on it again since last Saturday. How long can I keep going on and off the Unirod?

I had wanted Scheriproct suppositories initially as I had used this years ago and found it brilliant but the GP keeps giving me Unirod which I don’t think is working as well?!.

Been using the rectogesic since Dec. Had been doing twice a day initially, dropped to once a day when I thought it was getting better and forgot to do it a couple of days but am now back on twice a day. So it’s been about 8 weeks at this point.

When I pull my anus open to apply the rectogesic I can actually see a thin red raw line where the pain is coming from which looks kind of like a paper cut and also this same line actively bleeding in Dec so I am certain this is the source of the pain. Although it doesn’t look like any of the pics of a fissure I have seen online. it’s just a thin red line where as online the pics seem to show a rounded section at the end of the fissures?! So I am panicking slightly that it isn’t a fissure but the symptoms all fit….

What does everyone else use creams wise long terms as I can’t keep using steroid creams surely? Has anyone else had more success with Scheriproct than Unirod? I have called the doc again today and I have been trying sitz baths which do def help. I used Movicol for a week a while back but I am not really constipated I go every day once a day sometimes twice but my stool is quite large and solid so whilst it made the stool soft I didn’t feel like it agreed with me - gave me lots of gas etc. Are there any other stool softeners people could recommend I try instead?

Hi there، sorry you are suffering with this for so long. I suggest you go see a colorectal specialist because the GP is treating you with steroid ointments which thin the skin and prevent healing. They are good for hemorrhoids but not fissures. Take care and let us know how you get on.
8/16-12/16: Fissure due to antibiotics
5/17: Botox to sphincter, fissure healed
9/19: Trigger point injections and pudendal nerve block
11/19: Botox to pelvic floor
8/20: Botox to pelvic floor in new location.
On and off in pelvic physical therapy
