Need advice on how to reduce pain

This is my second time with anal fissures. The first time, I used miralax and recovered in about a week. This time it has been going on for three months (since mid January). I got a botox injection last week and it seems to have helped. Also using psyllium powder. BM's are not as painful as before but I am still in pain afterwards, usually for 3 or 4 hours. The ointment that was prescribed (Nifedipine with lidocaine) seems to irritate and make things worse so I have stopped using it. Do you know of any topical treatment to reduce pain/irritation? I have a lot of sympathy for all of you. this has been the most painful thing I have endured. When I told my doctor that each bowel movement hurt worse than rupturing my achilles tendon, he seemed surprised. I was not exaggerating.

Hi dhc, I’d use something mild like zinc cream (diaper rash cream). It’s soothing and can help with irritation and itching.
It’s still early days after you had Botox. Give it some more time. The fissure still has to heal and that can take a little while.

I tried diaper rash cream last time and I did not like it. Made irritation worse. I have heard that it works for some people. Thanks for the encouragement and advice to give Botox more time! The advice posted here has been so helpful. Just knowing that others have gone through this and gotten better gives me hope.

Hello there

I cured my first anal fissure with botox too. It took about 3 months for me to be completely pain free, and about a year after that to get rid of funny sensations, itching and irritation. So give it time and always keep those BM soft :-)
- Fissure developed in Jan '13
- Started rectogesic in Feb '13 and diltiazem Apr '13.
- Got botox Jun '13
- Healed by Okt '13, although I still had some irritation for about a year.
- New fissure April '22, healed June 2022
- New fissure 24. December 2023

Thanks for the encouragement! How did you keep the BM soft? I am taking psyllium powder and it seems to be working. However, not sure how soft it should be. Some commenters state that it should not be too soft. It should be well formed and not runny. Is that what you found? I will try to be patient. 3 more months of pain is a long time but I don't see another alternative. Thanks again.

Hi there

Yeah, I noticed some people recommend keeping your stools big and formed. However, I totally disagree. At least from my personal experience.
I do my healing best with a bristol 6, and only months after my fissure healed, I stopped the stoolsofteners slowly. Every retear I had was caused by a formed bristol 4 BM, so Im not buying the idea to cut the softeners as along as we are not healed.

I use magnesia, psyllium husk and lactose. I tried miralax, which worked great, but my IBS couldn't tolerate it.

Ideally you want to have a BM where there is no need to push. Just sit down, relax and let it drop by itself.

Status @ day 18

Had a few okish days without retears. BMs are once a day and bristol 6.

Currently taking 500mg magnesia, 1 tbsp psyllium husk, 15ml lactulose. Drinking around 3.5 - 4 liters of water. Staying away from meat besides chicken and fish. IBS complaining about the lactulose, but not too bad at the moment.

Will continue on just rectogesic for now, and hope thats enough for healing this time around.
