Need Help Please

So I said on my previous post I have 5 fissures for which the specialist gave me Diltiazem.

My problem is, as well as the fissures, I have a 2nd really painful problem and that is red raw skin around the bum. Since using this it has really aggravated it again (it has been going on since my fissures started about 8 months ago)

I have tried everything to clear up the red rawness such as steroids/hydrocortisone/anti bacterial/Vaseline/antibiotics over the past 8 months.

Sadly when I Diltiazem my butt blisters and when I stop the fissure starts coming back but blisters start clearing. I'm in a catch 22 situation.

Any ideas? I'm at my wits end....docs cant work it out/specialist thought this might help - it hasn't. Called specialist 4 days ago - no reply. :-(

Hi mountainman, I feel for you. So sorry you are in so much pain. I think you developed an allergic reaction to the Diltiazem,either because you were already irritated or because you can not tolerate the cream. I think you should stop using it, but please ask your doctor first.
I can relate to the irritation I had a fissure and the anal irritation was a constant in my bum even when I healed. I can not imagine if you have 5 AF how bad your irritation is! You have other options that you can try like Nifedipine or Nitro. Nitro has a bad reputation because it gives you bad headaches, but my experience was very positive. I cured my fissure with nitro. I have migraines, so I was very reluctant to use it. But my CRS told me to use a tiny amount about the size of a pea twice a day and oce I had acostumed to the oinment I could use more. I did it an never had a problem with headaches or other symptoms and my fissure healed well.
So you can try any of these ointments to see if they can help you. In addition you know you have to keep your BMs sof, have more fiber and drink a lot of water. Try to stick with one BM a day if you can.
I ended having a LIS, for another reasons (Not because my fissure didn't heal). That was 12 days ago and I am very happy with the outcome. I hope you can find a solution so you can heal soon. Best of luck and a lot of blessings.

Have you tried a nappy rash cream?? Some sudocream or bepanthen?? I don't know if it's worth a try.

Hi Mountainman,
I had a very bad experience with Diltiazem also. I got an itchy, weeping rash with it. I stopped using it and within 2 days the itch went away (the rash took longer to resolve).

I strongly recommend stopping the diltiazem. My rash spread over my whole body before I stopped using it. It's a rare but not unknown complication of dilt.

Ask your doctor for nitroglycerine. You can also try nifedipine if nitro gives you headaches.

Like BettyBlue, I healed with nitroglycerine. It did work, and it did not give me headaches. I could feel a bit of pressure in my head, but no headache, and it soon went away - 10 minutes max after applying the nitro. I started with much smaller amounts than Betty - more lentil than pea-sized, and gradually increased the amount over a week.

Good luck!
Nifedipine/lidocaine, no help
Diltiazem, effective, but caused major rash
Nitroglycerine, effective.
Topical estrogen for final healing.
Gentle heat to bottom - pain relief, muscle relaxant
Kondremul mineral oil
Time - lots of time.
Status - Healed!
