I'm sorry you are in so much pain. What do you mean "D"? Do you mean diarrhoea?
You'll hear different things about fibre. If your doctor knows you and is familiar with your IBS, he could be right - but most doctors would tell you to eat more fibre with a fissure. It's a bit controversial though.
The most important thing for you is to get regular soft but not liquid stools, not acidic, and well-formed. You might have to experiment quite a bit to find out what works. Keep a diary of what you eat and how your stools are.
There are lots of people on this forum who have IBS who will have advice to you. Also, do a search of this forum for IBS and see what you can find out.
You should also see a colorectal surgeon. They generally know a lot more about fissures than regular doctors. You should probably be prescribed nifedipine or diltiazem or nitroglycerine. These ointments relax the anal sphincter, ease the pain and increase blood supply to the anus to promote healing. Again, you may have to try more than one to see what works for you.
In the meantime, a painkiller like ibuprofen might help (if you can take that). Also, sitz baths, full baths and/or sitting on a warm hot water bottle can be soothing and help heal you.
