My diltiazem states on the tube to massage about 1 inch in and around the anus !
I find best results when I put the cream inside with the applicator it allows the cream to go higher up inside..

Yes, I think it is best to get the cream inside. My Mayo CRS told me to get it up at least 1 cm which I think is about an inch. Good luck and let us know how it goes. M

One "cm" is little bit less than half of an inch. My fissure was so painful and my sphincter so tight - there was no way of putting anything in. Even baby glycerin suppository could not go in and even if I succeeded it would end up "ejected" out LOL as soon as I would stand up - sorry for "visuals" - but some sphincters are just THAT tight and strong
