The pain after my fissurectomy surgery is giving me muscle spasms and my anus has tightened because of spasms I had in the past. What type of medicine can I take or what type of cream can I put down there that would help me to not have muscle spasms as much because the more my anus tightens the harder it is for me to pass a stool without straining even if I'm not constipated at all. Thanks for support.
Misdiagnosed with hemorrhoids in summer and fall '14
Diagnosed with an anal fissure in around Sept '14
Nifedipine cream since late '14
Tried botox around Oct/Nov '14
Hemorrhoid mistaken for skin tag until early '15
Fissurectomy April 1st '15

Hi Laban. Sorry to hear you are having muscle spasms. Do you have any nifedipine ointment to use (or does it not work for you)? If it's not working you could try another one like Diltiazem or Nitroglycerin. The best thing I found for muscle spasms was a warm bath. Some people have even had their BM in the bath or a sitz bath as it helps to relax the muscles.

When did you have your surgery?

Feb '15 Deroofing of recurrent infection from LIS
summer '15-healed but still ongoing muscle dysfunction/pain

I had the surgery yesterday and I do have nifedipine. It helps a little but I can't touch that area yet. I heard you had to wait a few days or so after surgery.

It may be too early for the warm bath then. Give it a couple more days - the first days after surgery can be very rough - I am sorry! Have you got painkillers? Ibuprofen works well for me.

When the initial pain and swelling has subsided, msimons suggestions are good, and I second them whole heartedly.
Nifedipine/lidocaine, no help
Diltiazem, effective, but caused major rash
Nitroglycerine, effective.
Topical estrogen for final healing.
Gentle heat to bottom - pain relief, muscle relaxant
Kondremul mineral oil
Time - lots of time.
Status - Healed!

I'm taking hydrocodone for pain. But even with the botox I still strain a little when having to pass gas. Is that normal? I still haven't had my first BM and now I'm going on day 2. I tried eating last night but the anesthesia from surgery made me throw up. I haven't eaten today yet. Is it normal to strain during BM even after botox?

The hydrocodone is extremely constipating. Did they give you anything to combat that?

I think they told me to take mirlax. But is there a pain killer just as good that isn't constipation at all? What would you suggest?

i have not had the surgery so i dont know what your pain level would be. I do know that hydrocodone is a powerful morphine like painkiller that is constipating, so might be good to combat that to get things moving. Also I have never taken Miralax either but many folks say it works well, although I dont think it works right away - takes a few days, you might need something that works quicker? Just my 2 cents, I wish you well in your healing.

I just don't want to be constipated. It's bad enough because my anus is tight from all the spasms from pain in the past making it harder for me to pass a stool without straining to make my anus less tight.

Laban, I agree with Scientist re: the baths. You should probably avoid much heat until 3 days have passed, otherwise you may make more inflammation and, therefore, more pain. But definitely after that, I would have lots of warms baths throughout the day to promote cleanliness, healing and relaxation.

Hydrocodone is indeed constipating and I would highly recommend you take Miralax. In the short term to get things going you could use Milk of Magnesia. I also second the reccomendation for ibuprofen. It will help with the inflammation and pain, which may help with muscle spasms.

Did you get botox at the same time as the fissurectomy? If so, botox, it can take up to 2 weeks to start working. I would usually start to feel in in 5-7 days though, so hang in there. It may be rough until it starts to kick in. Hope you feel better soon.
