I've finally decided to share my story and hope that I might get some advice and thoughts from others on this site. I'm slightly worried I might be made more anxious by replies I might get, so please, treat me kindly!
My anal fissure story began last summer (2014) - misdiagnosed as hemmorhoids, although I do have those, too, I eventually got a referral to a specialist who confirmed a fissure under EUA and administered Botox end Aug 2014. I had a follow-up colonoscopy beginning October which fortunately confirmed all to be well, and the same specialist banded some (between 1-3, I'm not sure) hemmorrhoids, and at that point said the fissure seemed to be healing well. At this point, I, too, felt that healing was well under way going by the reduction in pain, but a few weeks later I was in pain again. I saw the specialist again early Nov - who visited me in the ward about 30 mins before going under the aneasthetic, made a me feel fraudulent to be bothering him again, said that he'd have a look and if needs be do another round of Botox, but then " that would be it!". So, I'm guessing the fissure had reopened, because he did give me Botox again, but to be honest his manner was very abrupt, information about the healing path and duration was non-existent, and I came away vowing I never wanted to see him again and fed up with the whole thing. There was no follow-up appointment scheduled with my consultant after any of these visits - the impression he gave was one of job done, you'll be grand.
Where am I now? Well a year on from first Botox, things have improved, but I still have discomfort most days. I can carry on my daily activities more or less but certain things are painful, such as sitting too long on hard chairs, standing for too long, although walking around is ok mostly. I can't imagine riding a bike. With all the useful advice found on this forum, my diet is such that 99% of the time my bowel movements are pain-free and I have no blood - in fact I never had much pain with bowel movements or blood anyway. Overall the pain levels are low 1-3, I'd say, but I can't believe I'll have to live with this for the rest of my life. Two distinguishable discomfort sensations are one which feels like an internal muscular twinge in the region I believed my fissure to be, which I feel when I move in certain ways - it's not constant; the second is a general soreness which feels more like hemmorhoid pain and comes from a position closer to the entrance to the anus, but still internal -sometimes, burning, sometimes some pressure, not really itchy - and the level of this discomfort varies from day to day. I have perhaps a week or two of feeling I've kicked this thing, only then to have the niggles return more strongly without any obvious reason.
My dilemma: Should I revisit my consultant to check all ok (shudder)? Am I just on the long road to recovery - still? What is this twingey internal pain I feel when I do certain movements? Are internal hemmorhoids now the main source of my discomfort?

Personally, I would not see the consultant again. These things can take a long time to heal; there it is. I don't think I will ever be 'normal' again. If you think you need to see a CRS, if it is possible I would see one privately. Don't know much about the system where you are (Ireland at a guess :) ) but if that's possible, that is what iwould do. If you look On anniebee's post, you will see what helped for me!

For being so honest about it. I did wonder if this might be something I just have to live with and hopefully in time things will improve. I guess, in the absence of having had a follow-up appointment where the consultant has said " yes, the fissure is healed, the hemmorhoids are at this position, you might feel pain from this place and this place in the weeks/months/years to come, treat your body kindly and things will improve, if not this is the next option", I don't feel i've got a satisfactory understanding of where I'm at healing-wise.
I did have my three procedures privately in Dublin, and whilst my medical insurance plan at the time didn't cover all of the costs it did expediate getting to see a specialist quickly. Of that I was fortunate, and I could pursue this route again. Whilst I am loathe to put myself through an EUA again, or pay the costs, for peace of mind I might seek an appointment with a surgeon more generous with their time, who maybe offers the possibility of a follow-up in the office to discuss what was found and recovery plan.
For now, if things continue as they are and pain and duration of the relapses don't escalate I will see how I go.

You are very welcome. We are here to help where we can. I know it's hard to say, but 1-3 on the 'pain scale' is good (if you can say that about pain) and is probably what I live with some - but not all - days. At least you do not have the dreaded spasms for hour after hour (worse than childbirth, in my opinion) after a BM.
