Hi there, I suggest you go see a pain management specialist and get evaluated for pudendal nerve irritation. Often that can be the cause of burning while sitting. A pelvic floor physical therapist would also be a good option to evaluate the issue. Applying too many things to the area can also cause irritation. If something burns, don't apply it because it'll only continue to aggravate the tissue.
8/16-12/16: Fissure due to antibiotics
5/17: Botox to sphincter, fissure healed
9/19: Trigger point injections and pudendal nerve block
11/19: Botox to pelvic floor
8/20: Botox to pelvic floor in new location.
On and off in pelvic physical therapy

Yes I thought of nerve irritation and will be sure to ask crs the problem only occurs after bm and then lasts all day it returns after next mornings bm but before then I am fine for a little bit before I go in the morning trying get things done in that short window. No ointments ever help even for a short time thank you!!

Dr was great the burning is from the tightness or spasm of the sphincter he was able to elicit the burning by putting pressure on my sphincter the 3to 4mm shallow fissure sharply stung when he touched it but it didn’t burn per se enlightening exam advised to continue nifedipine lidocaine fiber softener lots of water for another six weeks he said sitz if they help with cleaning or for comfort alternating Tylenol aleve Suggested Botox if necessary but would not be comfortable with a spincterotomy given my previous sphincter repair Any one in Rochester my area I highly recommend Rochester crs and dr teng
Fingers crossed I can heal trying not to worry is hardest thing thanks!!

It's great that your doctor visit went well. I truly hope you will heal well with this regimen. Take care.

Help I healed in June of 2020 but was diagnosed with levator ani I had a colonoscopy and Botox 100 units on each side at the Cleveland clinic January 20 a total failure !!! I now am worse I have a weighty feeling when I walk never had this dr thinks it’s the colonoscopy aggravated adhesions on my sigmoid colon from hysterectomy or a sigmoidecele he even carelessly suggested I could have it removed ? I could not believe this is he crazy I am now worse I have weighty feeling burning sitting still swollen internal and external hemorrhoid tailbone pain when I sit or have to have a bowel movement and a feeling that I am constipated or cannot expel movements even though I go once every morning. By the end of the day it feels like there is stool filling up and stuck in there I never had any tests or even a proper exam by this Cleveland clini. Dr he is retiring and is referring me to a colleague but to wait six more weeks for the Botox to wear off I did have pelvic floor pt and Valium before this on the suggestion of another crs that helped so I think I may ask pt for advice but does any one here have experience with levator ani botox??? I cry every day please help if you have time thank you

FYI colonoscopy was done here because I had a failed one and ct colonoscopy locally due to a torturous sigmoid colon and suspected polyps I had none!!! The Cleveland clinic dr didn’t even do a Dre or any tests either I now think this was wrong o thought I was getting better treatment at the world famous clinic but no I don’t think I did I am lost please help

Have you tried taking magnesium citrate supplement? I suffered with fissures on and off for 10 years or more and had various creams from the GP. I was referred to a consultant years ago, but in the meantime had read on this forum that magnesium supplements could help relax the muscles and ease the spasms I was getting. As soon as I started taking the magnesium citrate the spasms eased and the healing process began. Fortunately it meant that I didn't have to go through with any surgery. I have continued to take these supplements ever since and if I run out and don't take them for a while, I start to get 'twinges' and take that as a warning sign and start taking them again. They may not work for everyone, but they are certainly worth a try if there's a chance they'll help. I swear by magnesium, it has totally changed my life and apart from the odd 'twinge' when I forget to take the magnesium, touch wood, I've been problem free since.
I think magnesium citrate is the better supplement of the magnesiums to be effective in this case. I take Solgate Magnesium Citrate tablets daily (200mg of magnesium each tablet, so 400mg a day) and easily available from Health Food Shops or via Amazon. I still take them as a daily supplement and am fortunate that my problem does not reoccur whilst I take magnesium citrate. Wishing you well!
