Help with 7 year Chronic Anal Fissure

I've had an anal fissure since I was 13. I am 20 now. It took me a few years to actually realize it was a fissure that was causing my problems. OK, so my case is probably very abnormal. It's not a normal fissure. I believe I got it when I put a pencil in my butt.. I dont know why I did it, I was curious. As I am a male this is a very embarrassing topic for me. I believe it tore my sphincter pretty severely. I can feel the skin tag down there. So it was not just a large stool or whatever that caused this.
So here are my symptoms: I cannot stay clean down there. I just CANNOT do it. No matter how many times I wipe, there is always more showing up. This causes me to stink, and is the main reason I'm desperate for help. Also, I experienced chronic diarrhea for many years. 75% of my bowel movements were loose, runny stools. This past year it has been better, but not great. Also, I would experience a drop of blood on my toilet paper every once in a while.
This has gone on for long enough. I dont have health insurance, I cant turn to my parents due to embarrassment, I'm really just lost. I dont want to do the safe options, like put creams and stuff on there. I know it will help, but it wont cure it. I just want it to be cured once and for all. Is that even possible? What doctor do I go to? Can I just opt for surgery right away? How much approximately will surgery cost me with no insurance? I have up to $3000 to spend. I'm tired of being the smelly guy. I need help, but it seems almost overwhelming. SEVEN years i've had this. I cant take it anymore. Depression has plagued me for the past 5 years. Lost in Thought, please help.

Lost in Thought,hi there, sorry to hear your are having this problem for so long, but most here in this forums knows how it is to deal with fissure for a long term.
Because your fissure is now chronic due to the long term suffering of years, the only thing to do is to see a Colon Rectal Surgeon, now to keep the area clean (trust me I know how it is when you can not clean the area enough), do sitz bath, before a BM lubricate with vaseline or any organic oil, (no perfumes or scent), then the sitz bath, you can fill the bath tub with the warmest water you can handle and sit for a while and relax.
Also you can use the shower head and spray the anal area with it for while after a BM.
In my case I found great relief taking a Q-tip with vaseling and Gently wipe the inside of the anus but not to far inside (and only if is comfortable for you to do so), until i don't see any more stains, this helps with the long day irritation of having left over fecal matter in the area.
Also do no use toilet paper and use baby wipes they are great to clean the area and refresh.
Now while all this things will help and you will find many suggestions in this site, you have to see a CRS as soon as possible as chronic fissures tend to get worst as the years go by without treatment.
Do not be embarrassed to tell you parents that you have developed a fissure if they can help you with the cost of seen a doctor.

I'm so sorry you feel so alone and helpless with all this but you really have found the best place to be with this site. There are so many people that are here to offer you so much support and advice.
It doesn't matter how your fissure started - We all do incredibly stupid things to ourselves. No one is going to judge you here and when you get the help you need (because you HAVE to get this help to get yourself through this) you don't have to reveal that fact. Fissures are triggered by numerous things. The main thing is to get it sorted. I can't offer any advice on costs in the US as I'm in the UK but I'm sure someone can.
If you feel you can't tell your parents right now maybe you have a close relation or someone you can confide in??? Please don't keep this to yourself and don't dismiss the creams and aids that can provide you immense relief with a fissure....
Cleaning can be a tricky issue and I know this only too well myself but I find the best method is to use a shower head after every BM to clean off and in the morning, I always hop into a bath after doing that to let the water get into the anal area and then I shower the area off again. In the evenings I always have another bath because it really helps me feel and stay clean. You don't need to use soap just lots of nice warm water. In the daytime, if I suspect any collected debris in the area, I have a pack of baby wipes in my bag and I just pop to the loo and pat the area with a wipe to ensure it is clean.
You NEED to see a GP to discuss this and your depression connected to it. 20 is too young to be going through this! You have a whole world to discover and relationships to form and please don't deny yourself these opportunities because of your bottom!! Just taking that bold step to tell someone will start the ball rolling to address the issue and hopefully put you on track to getting the help you need.
You will need to see a CRS (the anal specialists!) and it will take time to figure out what works for you and the course of treatment you will need....but honestly, once you start taking these steps to help yourself, you will feel relieved that you are taking action, relieved you have taken some of the weight off your own shoulders and you will start to regain your confidence as you start to get better and better....All those smell issues will start to disappear and you'll probably kick yourself that it took so long for you to do it!!.......
Don't lose yourself in thought any longer, lose yourself in acting as soon as you can and the determination to sort yourself out!

oky lost i am sorry to hear that u in pain but the help is there. i do not know where you live but in new york we have state sponsored health insurance in before 18 you are qualified for a heath insurance. not a big deal with pencil or with large stool its not from the pressure its from a trauma very well surgeons can sew it not a big deal it has not become fistula yet. fistula is greater problem. hurry to the gi he will refer to the surgeon or colonscopy deal they will see what its inside then they will decide to proceed i am wonder why u suffer so much.for nothhing......
