Not "presenting" as a classic fissure

Postby hootinannie » 04 Oct 2013, 01:09

So, my latest visit with the CRS was interesting...to say the least. I've had about 4 visits and at none of these visits was the dr. able to properly examine me--for the usual locked-out sphincter reasons. Yesterday, I was examined thoroughly. Ouch.

Findings were not what he or I expected. After being lectured thoroughly about the source of anal pain and how it almost always stems from one source and not several, I was informed that my rectum looks like a war zone. Words to describe it were "not presenting as a typical fissure", "very raw with several areas of inflamed and torn tissue". CRS was unable to pinpoint if fissure, hemorroid or just plain tissue damage was the source of my pain. After being very informative and forthcoming and talkative at the start of the appointment, the Dr. looked kind of shocked and worried about what he saw. He wrote me a prescription for very strong steroid cream and is referring me to a surgeon--yes, I'm going to a CRS who no longer performs the surgery that he thinks I need, bummer. He also said that he didn't think the surgeon would be very happy to see the condition of my bum, which scared me but I was in such shock I didn't ask questions. I'm not to drink caffeine or alcohol or eat chocolate. Lots of water, and not to overdo the soluble fiber. The only thing he can recommend for pain is Tramadol, and even that must be used sparingly, as it can cause spasms.

So I've been treating a fissure for several months and this looks like it might be more serious than that. I just don't understand why this happened to me. Sure, I've been constipated in the past, but what on earth is causing this to happen to me? I did have banding done about 18 months ago, could it be that it was not done very well? Does anyone have experience with this type of diagnosis? I spent hours searching the web tonight, but can't find too much.

Ugh...I am growing very, very weary...

Hi Annie,
Was it actually a CRS you saw? Sorry, I got a little confused as you switched a little in your post to referring to him as a CRS and then a Dr??
At this stage, I would say try not to stress too much about what has been said to you. No disrespect to him intended, but if it was a GP you saw, they are not specialists like CRS' and it is possible that he has not had a lot of experience in dealing with people's backsides, so what he saw in you may be nothing worse than in any other anal sufferer. I had a GP who quite clearly, had no experience in fistulas and diagnosed it as a Pilonidol Sinus at the time and his nurse told me it was a superficial skin tag and he panicked too when he saw it, because to be quite honest, I'm not convinced he had ever dealt with one before!! So please hang on in there for the surgeon's (CRS' ?) opinion....
TBH, the more I read your post, the more I actually feel quite uncomfortable about what he has said to you and I'd be glad to be seeing someone else! I don't think the appointment was that helpful or reassuring and no wonder you are left feeling confused & a bit scared!.....It actually sounds quite unprofessional.
It could be that the banding has however left you with some issues, possibly unresolved, as that can happen and it's good you will be getting a proper check. You wouldn't be the first person on this site where hemmie removal can lead to further issues. You sound like you are doing all the right things, so just hang in there and take the next step, see what happens and if you are still confused or have questions, perhaps seek another opinion???
There are a few here that have had issues following hem procedures, so hopefully someone will add to this post soon.......

Yes, he is a CRS, but unfortunately does only certain procedures and refers others out. He was originally going to refer me for LIS but I'm not sure that is going to happen, given his latest "diagnosis". I am going to another surgeon, hopefully one that will be able to help me. I would really like to hear from others who have had issues after banding, because I think this all stems from that procedure, unfortunately. And thank you for the feedback. This forum rocks!
