Ah could you do some biofeedback I know Gilmore girl has found it helps, I have never looked into it as I very rarely if ever need to strain, but maybe if your going to have this issue every month it might be useful for you. You can get referred to clinics on the NHS.
I just had mine in the hospital and like Katie said a tube is put up your bottom....but if you get a really good surgeon they can tell just by trying to insert a finger....my current CRS just had a look at my butt while asking me to cough and he got my pressure the exact same as the test did....he doesn't agree with internal examinations unless there is a concern about something serious.

I would say I'm about a 4 at the moment which isn't bad since as I had a tear this morning, it's strange when I had Botox a year ago I was in no pain when going to the toilet, I didn't need to strain either but this time round It still hurts going and after and I'm so sore all the time

I guess I'm in pain but only a 3 for a few hours after a BM it's the burning pain so not really an issue especially not after spasm pain.
Becka I know this might sound a bit strange but could you leave it till the very last second to go to the toilet...I mean like don't sit down until your worried about your pant...I know that might be TMI but that is what my CRS told me to do as it takes any pressure off my hems...I have only been doing it two weeks and it has helped. Not sure if it will help with your straining but it's worth a try, especially of it's not something you need to do all the time
