Not New to Fissures. New to this one!

Hello all.

New to the forum here but not new to fissures. I started getting fissures around 5 years ago. Kinda figured out what they were when I had my wife take a look. They never affected my life much. A few days to a week of pinching when going to the bathroom, sometimes a blood streak and my day moved on. I think my worst ever was maybe 2 weeks of some uncomfortable bowl movements and maybe 10 mins of discomfort after. They never got me down. I even was able to joke about them.

Right now I am dealing with something I have never dealt with before. It has taken over my life for the last 2 weeks. I can see the fissure in a mirror when squatting. It is huge and deep! I will feel fine then as soon as a turd comes close to my anus the whole entire left side of my anus goes nuts. It spasms and it feels like im ripping everything open. The pain is unbearable. And the turd is pretty soft and narrow. It feels like its the size of a submarine.

Heres the problem. I have always been confused about my fissures. I have never been constipated a day in my life, I eat a diet better than anyone I know, I am extremely active. (run 20-30 miles a week, bicycle, hike, snowshoe). My stools are what I believe to be always soft. The only thing I can think of is stress? And now im so worked up with this one that my spinchter goes into a panic when I go to have a bowl movement. I am 39 years old male. I am in one of the most stressful periods of my life. Im used to a really laid back stress free life. Going through some things right now.

I have a HUGE fear of doctors. This is so bad I just scheduled an appointment for a colon rectal surgeon (im not messing around. At this point I know more about these than a general doctor would so im going right to a specialist). I am hoping at 2 weeks I am catching this early enough to be fixed but I am really nervous how deep this thing is. I feel like the doctor is gonna be blown away by it. (hoping not). Anyone on here able to see their own fissure and how large it was or is?

Any words of comfort would be great right now. Im feeling pretty down about this.

I am sorry you are going through this! I am totally new to fissures and it's been unbearable. Missed a week of work and don't feel any closer to getting back. I did go to the doctors and will say it helps to know exactly what you are dealing with. I thought I had an abccess but found out it was a fissure and got Rectiv for it. It's not comfortable (I didn't find it fun) to put yourself out there for inspection but it's over soon enough and then you can get help on healing! Good luck!!!

Good that you are going to a CRS! Yes, fissures can be unbelievably painful. I'm sure the doctor will have seen fissures just like it or worse, so don't worry about that! Sounds like you are doing lots of good things. Hot baths may help the spasms and you can get a portable bidet to help clean up. Also try hot water bottles, ibuprofen. The doctor will be able to give you an ointment to help. Hang in there!! It will get better and going to the doctor is the first step. Keep us posted!

Welcome to the forum. There is a lot of great advice here and some of the kindest people too. I hope you get in to see you CRS soon and that he/she is gentle but thorough with you and that they have good plan to help you out. Keep us posted on how things go for you.

Thanks for the kind words.

I sure hope something good comes out of this. Like I said, I have had some fissures but this one is insane. There is absolutely nothing sold over the counter that really helps. I need to get my self to stop spasming and im truly hoping a good prescription cream can do the trick. I can feel fine like I don't even have anything going on down there, even feel how relaxed the sphincter is and as soon as the bowl movement gets to a certain spot...Bam! Tightening and pain like I have never felt. then 80% of the time the whole entire sphincter will spasm all day. There is no way possible for it to heal if my sphincter continues to do that. Im truly starting to believe I allowed this to get in my head and I am creating the spasms. I never locked up like this before with other fissures.

Could just be a particularly bad one. Spasm of the internal sphincter really isn't in the head. Anxiety will make it tighter but not spasm. Don't blame yourself for that. What Country do you live in?

I have that bam tightening thing too. And I've had LIS! The part that wasn't cut can still do that I guess...

Have you tried soaking in a warm bath? Heat really helps relax the area and increase blood flow for healing.

I am from the US msimon. Yes I am all too familiar with the sitz bath. Im not looking for relief from this I am looking for a plan to get this healed. The sharp pain and extreme tightness feeling has not let up in over a week. A little more than a week ago I had a day where I thought there was a turnaround. Had a bowl movement that was nothing more then a very sharp pinch (no extreme tightness) but right after that bowl movement was my absolute worst day of spasming and pain and since then everything actually got worse. When I go it feels like I am opening up a bullet wound.

Frank-have you been able to get to your doctor? Or maybe go to urgent care?
Sounds like the pain is urgent. I have been in pain for weeks but last Tuesday it was so bad I made sure I went to rectal doctor who prescribed Rectiv. They recommend three weeks to know if it's working before trying anything else and I week into it I feel better. Still not able to sit for long periods of time so not at work yet but I can walk and lay down not in pain! Rectiv might help you too.

Went to the CRS today. She had a look at it and right away said "yah thats a fissure alright". She said it was deep into the muscle and claims I have had it much longer than 2.5 weeks. I wanted to disagree with her but didn't say anything. She prescribed a cream (cant remember the name but she claims that is not the one that gives headaches) unfortunately I have to wait until Friday for it. She right away said it might look like surgery for this but said I have a 50/50 chance of healing it with the cream. She wants to see me in 10 days. I find that a bit quick to see if anything is better.
