6 weeks post Botox - still so much pain - has Botox failed?

Hello everyone,
Long time lurker here - 16 year fissure sufferer (can't quite believe it's been that long). Anyway, managed my fissure over the years with home treatments and diet, have had some very dark days over the years buts managed somehow.
I feel so much for anyone going through this horrific pain and am so grateful to this board as it has really helped me not feel so alone over the years.
6 weeks ago I had a fissurectomy + Botox injections for a right side anterior fissure (came about due to childbirth). The surgeon said that he also did a bit of an anal stretch and cut out scar tissue from the area, removed sentinel pile and cauterized the wound.
It has been a really hard 6 weeks and I am still in a lot of pain - both at time of bowel movement and for hours afterwards with spasms. I have been following all the post surgical instructions applying the nifedipine cream several times a day, taking sitz baths, taking MiraLAX daily, drinking lots of water, eating fiber, etc.
Just looking for opinions as to whether the Botox treatment is considered to have failed at this point as I seem to have no relief from the sphincter spasms. I am really getting desperate at this point as I feel I'm in a worse place than before the procedure. Before I could get back to a place of relative comfort by being very strict with diet and home treatments within a couple weeks and have a bit of a reprieve from the pain, now I have had a constant six weeks of pain since the surgery and other than the bleeding stopping, there is no sign of improvement with the pain.
I don't know if I should contact the surgeon and see what he thinks. Contacted him at 4 weeks after no reprieve and he urged patience, which was so hard to hear. I had been to another specialist a few years ago and he stormed out of the room and left me in tears after I had asked about LIS, so now I'm quite scared to ask about it again, although this is a different doctor.
Really struggling and not sure what to do next.. Has anyone else had Botox that worked after a long time? Any guidance anyone can offer would be so appreciated. Thank you!
