People with IBS tend to have hypersensitivity throughout the GI tract. So, if you feel the urge like you have to have a BM and you go to the toilet and nothing comes out, or maybe just some gas, it could be that there is a small amount of stool in the rectum, enough for you to feel and to create an urge (particularly if you have gas behind the stool), but not enough stool to actually produce a BM. In a "normal" person without IBS, the sensitivity in the rectum is not that touchy and will only produce the urge to have a BM when there is enough stool in the rectum to be evacuated. In those with constipation problems, it can be the opposite problem. A rectum that is not sensitive enough to the presence of stool.
No doubt having had fissures and other anal problems has just heightened this hypersensitivity even more.

Jr2,
That's exactly what I fear is happening - you've pretty much summarised my situation completely & like Guiness, I think this might have been the root cause of my fissure too originally as I've never had major issues with constipation. I've always believed in that mind/ body connection and a stressed out mind has to have some physical effect or release somehow if not treated or dealt with accordingly. I think enough time spent on stressing about the fissure itself....it's about time the focus went on the destressing of life events generally...something to work on for sure right now....

wow jr2, thank you for the explanation, that pretty much explains my problem. uhmm now I believe even more I have IBS. Or at least that hypersensitivity. Today I push until the last bit came out. I'm not feeling the sensation to go. But I'm in pain Image , no re-tear though.
I have read, that when you have this sensation you have to think that there is no such a thing as an empty bowel, and that you have to try to concentrate in something else. But this is more easy said than done.
What I like about this explanation is that maybe that bit is not meant to come out, so I have nothing to worry about, before I thought I was constipated, but the last day I remember I didn't have a bowel movement was 10 months ago, the day after my last c-section. So I'm not constipated, I'm might just be hypersensitive.

I'm convinced by Jnr's words too now that it's totally hypersensitivity going on with me as I did wonder about constipation a while ago but it's just not something I've ever had major issues with! I'm just going to have to change my thinking now about my toilet habits and relax a bit more in the knowledge that I probably simply don't need to go when I keep thinking I have to!! (which at the moment, is a lot!) Definitely going to work on being a lot more relaxed!!!
& No more "pushing"!... Sounds like you probably don't need to either anyway!....Your baby is so adorable!!!! :)
