yes correctly I see a general phycian.But still I am living with this pain
.Thanks to Lecia he gave me some link but Maybe I can not afford that diltianzem.Because I'm so far from USA.So I'm contemplating to see another pshycian.I have to my fissure cured in two moths.Because 3 moths later I will attend to military service.It would be so horrible before curing it to attend military.Coz I will away for maybe 6 or 12 moths away from my house.Maybe It would be impossible for me to find good doctor in military.

Yes, it would be very important to get this healed before your military service begins.
I think seeing a different doctor would be in order here. Tell him that you need to have this resolved before you leave for your service and mention the creams and email and see how much it would be to send you the cream if you doctor would prescribe it for you. It is quite possible that a pharmacy in your country could make the cream themselves. It is made out of a readily available blood pressure medicine that is compounded into a cream. The pharmacist where I get mine says it is really easy to make and he could even tell your pharmacist how to make it if you would like to call him.
There has to be someone there who has experience in treating this-I am sure you are not the only person who has gone through this!
While you are finding another doctor if you could make sure you are getting around 30 grams of fiber in your diet a day and drink 2 liters of water that will help to keep your BMs soft so that you don't further injure yourself. Also try to soak in the tub in warm water for 20 minutes after going to the bathroom and then at night before you retire. That will help with healing and with the pain.

Are you just having pain during and right after having a bowel movement or are you having pain that lasts for hours after you have a bowel movement?
If you are having pain that lasts for hours after having a bowel movement then I would strongly urge you to see a colorectal surgeon and consider having a surgical procedure called Lateral Internal Sphincterotomy. You can see one if you Google Proctosite and go to a video of lateral internal sphincterotomy. If you need this taken care of in 2 months then you really need to see a specialist that knows something about how to get this cured.
Please see a colorctal surgeon that has treated lots of Anal Fissures.
I am sure that there are good doctors over in your country.

Thanks for advise and wishes,I noticed that my pains lasts nearly 5 or 10 minutes after BM and there is no bleeding in tissue.I guess that it is not serious situation but I havent understood yet why it has not got cured It seems serious accoring to you?
In turkey I guess (but not sure) all the doctors responsible here are general phycists.

I'm not doing curretly for handling the pain but I 'm tring to abstrain from white bread.I guess It cause the constipation and hard stool is not it?And also some people here claimin persistenly about tea cause constipation.Last time I asked you but I wanna ask again.The tea ,Im talking about seems like ENGLISH 5 O'clock nothing more special.I mean something like Ceylon tea consist of just water and dried leaves of tea plant that is boiled.
Lastly In long term that I have described before cause any improvement or much more trouble if they dont cured by 3th element like medicines?
(but I'm still contemplating to see a good doctor.But I havent found enough time coz graduation thesis.)And what do you think about antibiotic medicices?Can be used in fissure treatment?

That is good that you don't have any bleeding. That is a sign tha you aren't damaging the tissue enough to make it bleed when you go to the bathroom.
You really need to make sure you get enough fiber-whole grains like whole grain cereals, breads, lots of vegetables and fruits.
The tea should not be constipating. In fact, caffeine can have the opposite effect. I am not sure if there is anything that can be irritating in tea- I would not think so though.
Antiobiotics probably would not help unless you have an infection. It would not do anything to cure the fissure. The best way to do what you can is to keep your bms soft. I don't know if you can find a mild stool softener there to take? I don't mind sending you some if you want to try it and see if it helps. It is not a prescription or anything so it is pretty mild. It is called colace and is used quite extensively for hard bms.
Once you get done with graduation (congratulations by the way) maybe you can see a different doctor or have the same one check and see how things are progressing and ask him about what you can do to keep the stool soft. And really think about taking baths too. The warmth from the water really helps with healing by relaxing the muscles and bringing more blood flow into the area.

If you jare not having any long term pain after the BM it may be that your condition is not so severe that it is causing your sphincter to go into spasm. If so, THAT IS GOOD! The typical way of treating Anal Fissure in the US, Canada, the UK, and Australia is to first try jto treat the condition medically with either nitro cream, nfedipine, or diltiazem . The Doctors will try those for any where from 6 weeks to three months. IF they have little or no success, they typically will then try either Botox injections or Lateral internal Sphincterotomy. All five of these treatment options are designed to relieve the tightness of your internal sphincter to allow for more blood supply to the area and thereby promote your body's ability to heal the fissure wound itself. Only occassionally will the surgeon actually work on the fissre itself. If they do they will either sew it up or graft some skin over it or remove what is known as a sentinel skin tag.

I just want to urge you to not let it drag on and if the pain is, or becomes too excruciating be sure and tell your Doctor. Mine was intending to try and treat me medically for several more weeks until I told him that the pain was completely interfering with my ability jto interact with people in a meaningful way or work . He then scheduled me for the surgery and once I got that done I have healed up remarkably well.

You had botox treatment or graft some skin?If the second one;did you felt any pain after surgery or during?
I'm planing the worst situation,is that surgery so I wonder whether it is painful or not.Here I red about it,it is said surgery takes 15 minutes.It that right?And what is the cost in USA
I wanna reemphasize ,in my agenda ,to get heal it by medicine is first option but I cannot disregard the worst.

I did not have botox or skin graft. I did have a Lateral Internal sphincterotomy. You can see one being performed if you google procto site-go to the site. go to video atlas on the site and look up anal fissure. they have a video of a Brazilian Surgeon performing one.
I felt NO pain during the surgery and very little(in comparison to the anal fissure pain) for the next few days during my recovery
