Hey GG, it's good news that the surgeon thinks you're healing up well. Also good to hear that the bum pain is easing. I'm not sure that colorectal doctors know a lot about the pelvic floor (as well as other specialists). The first CRS I saw said the internal sphincter can't be helped much by pelvic physical therapy, a statement that had my pelvic physio say 'how idiotic' like a broken record. The sphincter doesn't just exist in isolation. It's surrounded by muscles that affect its state.
As for anxiety being the cause, that sounds like something a doctor says when they don't have an explanation for what's going wrong. People are generally intuitive about their own bodies and if you feel something besides 'anxiety' is the cause then they should help you find the answer.
Your symptoms remind me of stomach problems I had back in 2009. Every evening around 5 I would get a lot of gas and bloating. I don't remember what my BMs were like at the time, probably loose. This went on for a few weeks Eventually the problem went away, I thought it may have been amoeba, but now I think that I really don't know what it was.
8/16-12/16: Fissure due to antibiotics
5/17: Botox to sphincter, fissure healed
9/19: Trigger point injections and pudendal nerve block
11/19: Botox to pelvic floor
8/20: Botox to pelvic floor in new location.
On and off in pelvic physical therapy
