That's great that you're able to do all that mpb! But I know what you mean, it's so frustrating to be close, but still in pain.
Do you find the pilates aggravates things? It may not be true for you, but with any personal training clients with tight pelvic floor, I would avoid most core isolation exercises (especially ones where you sit a v, like in the "hundreds" move in pilates). It can really engage the pelvic floor, which is maybe not beneficial if the ultimate goal is to relax the muscles down there. Just a thought! If you're looking to do some core work, I could maybe help point you to some less "pelvic floor focused" exercises you could add in.
