Hi all,

For over a year I had been experiencing discomfort when going to the bathroom for a BM. It wasn't until passing a BM started to feel like I was passing glass or having a knife cutting me. I never had any blood on my toilet paper just pain and discomfort that seemed to last all day. I was always figeting after the BM, struggling to find a comfortable position to be in. Although I never could find a comfortable position the pain and discomfort would pass on its own only to return the next day after another bowel movement. After a little over a year of dealing with this the pain and discomfort only increased so my wife finally talked me into consulting a doctor and getting checked out. The doctor performed a manual check up and that was the first time I ever saw blood come from my butt but it wouldn't be the last. He prescribe anulsol suppositories and 2% lidocaine gel to treat for hemorrhoids. This treatment seemed to ease off some of the pain but not all of it. I started to notice more blood in my stool, and some times even have a drop or two fall into the toilet water. Concerned I went back to the doctor as not only was a experience blood in my stool my pain had double. He was concerned as internal hemorriods are normally painless. I was sent to see a specialist who was to perform an exam under Anastesia. While in there she found 2 internal hemorrhoids and 1 internal one which had been sucked back up into my butt. She performed a hemorrhoidectomy for the hemorrhiods and a spinctorotomy to relax my spinster as she felt it was too tight and the root cause of my problem (guess I have a new meaning to being a tight ass). Next entry I will talk about my "recovery" as thing post is getting very long

Surgery recovery

As I mentioned in my first post I had a hemorrhoidectomy and spinctorotomy done. The surgery it's self only took about an hour. I was in the recovery room for about 15 mins before awaking from my doctor induced slumber. Soon after I was drinking water, eating jello and speaking with the nurse as it is required before they would release me to my wife so that I can go home. The doctor had informed my wife while I had been out that this would be the most painful surgery recovery I will ever feel worse then giving birth (although I'm male so I will never be able to compare). We left the hospital armed with me and a handful of prescriptions which consisted of Percocet, adavan, 800 mg Motrin, and lidocaine gel 2.5%. The hospital was only a half hour drive from my home but we had to stop to submit the scripts and wait for them to be filled about an hour wait. So to put things into perspective an hour after leaving the hospital I was all ready starting to feel the pain. I sat quietly in my wife's car crying as I waited for her to return with my pills. I certainly didn't expect to be in pain so soon after the surgery. My wife did her best to rush me home so I could lay down. By the time I got home I had taken two Percocets and one adavan (muscle) relaxer. I was starting to feel a bit more relaxed so begins my time of a month and a half sleeping on an air mattress as I didn't want to disturb my wife at night or risk taking a knee to the butt while she slept. I had a steady soft diet of ice cream, jello, pudding and protein shakes (protein powder, almond milk, peanut butter, ice). Since I was taking narcotics I had to balance it out by taking 2 dulcolax stool softners and fiber drinks, about a gallon of water a day and an occasional ginger ale to settle the tummy when it wasn't happy about the morning pills. Like clock work I visit the bathroom every day and the day after surgery was no exception. It was quite painful, there was blood and it felt like my butt was ripped wide open by some blunt device. I was not prepared for that at all. I'm screamed because of the initial pain, but then because of the sheer burning and immediate swelling of colon. Jumping into the shower to let warms water run over the area seemed to bring down the pain slightly but not much. Pain management has gone out the window. I would spend hours in the tub or the shower trying to ease my pain. It was like I was experience the same symptoms I originally went to see the doctor for but the pain was 10x worse. So let's fast forward a month goes by of this and it's finally time for my first post op check up. I'm still in tremendous pain, and I have a new symptom as it has become very hard to have a bowel movement it takes a large amount of effort for me to pass a BM and with it extreme agony during and after the BM with lingering pain for hours afterwards. I told my doctor this yet she couldn't perform an exam due to the pain I was in. She was shocked to hear I was not feeling better, then decided to tell me i might part of a small percentage of people who will always be in pain. I was almost out of drugs, but that didn't seem to matter as they were now doing nothing to curve the pain the doctor also didn't offer to give me anything else. Next post will have information about my 2nd post op doctors visit and me returning to work.

My extreme pain and hard to pass BM followed by lots of blood pouring out of me continued and I had another follow up two weeks later. Again no exam was done and I was told again I might just be part of a small percentage of people always in pain. The pain has all ready defeated me, and this is something I never want to hear from a doctor. Your a damn doctor there is something seriously wrong with me FIX ME. Frustrated I left the office no better then when I walked in. At this time it had been 6 weeks, and it was time for me to try and go back to work. So I tried working past the pain, but if I had a BM at work all cards were off the table I had to leave and go home. Some times I could make a full day of work some times only half days and other times I just couldn't make it in at all. Going on 2 months of this and I break down and go to urgent care in hopes a different doctor would have mercy on me and give me something for the pain. A quick examination worried the doctor. He couldn't give a proper examination because I had formed anal stenosis ( narrowing of the butt due to scaring) he said my opening was about the size of a pencil. Well that explains why I have to push so hard to get my BMs out. He also thought he saw a fistula so he flat out told me to go to the ER. So I drive my self to the ER and get checked in. I'm seen by a surgeon who finally answers my prayers and gives me something for the pain. Deluaded given via IV it was great in secs I went from raging pain to high as a kite. For a brief moment I was at peace. He gave a 2nd opinion that i was in fact suffering from anal stenosis but he wasn't sold on the fistula so he ordered a CT scan and sure enough no fistula which was at least a little bit of good news. After spending 6 hours in the ER I was finally released with a prescription of delauded and a referral to a colorectal surgeon. A week later I was in his office for another exam, 3rd opinion yup it's anal stenosis. I had to back track and catch him up on how I ended up at his office. So when I described my original symptoms he immediately mentioned anal fissure which was the first time I had even heard of that. He scheduled me for an exam under Anastesia and anal dialation. 4 hours at a hospital for a 20 min procedure. By the time he got done with me the opening went from a number two pencil to being able to fit two fingers in my butt. The surgery was not bad and this doctor used a lot of local so that entire day I couldn't feel my butt at all ( it was amazing). Like clock work the next day I had a BM it was weird in so many ways. I didn't didn't have to exert myself to pass the stool it can out on its own. For the first time in 2 1/2 month I could literally feel my colon expand to allow the passing of the BM. It was amazing then I was hit by a massive burning sensation most likely due to the BM passing along the new tears in the scar tissue, but soaking in a warm bath relieved me of that burning pain and when I got out I was no longer in pain. No burning, no bleeding, no swelling I felt like a new man. Unfortunately this reprieve only lasted two days. Then slowly the pain and the swelling and the blood came back. Called the surgeon up and he recommended that I try manual dialation at home. I'm pretty sure you guys and gals can figure out what what means so I won't spell it out. But what I can tell you is that it's painful and has to be done twice a day.... Just to keep the opening from closing back up. After a week I returned to my original extreme pain and blood passing and swelling. Although it was much easier to pass the BM something else was causing this extreme pain. Possible a fissure but this surgeon was unsure because he said when he examined me and did anal dialation he found really bad scaring inside of me and in his 17 years as a doctor has never seen scaring like it. So he recommended I consult another specialist for the procedure anal advancement flap. He wanted to keep me in house and referred me to another surgeon there but he wouldn't have been able to consult with me until the 15th of this month. I have now been living with this pain for 4 months and that wasn't a viable option for me. So I called around and found another specialist that could see me sooner, and for a 2nd opinion on if this new surgery will fix me. He agreed that it was the best option. A week later oct 1st I was in for the surgery. Next post will begin talks about my anal advancement flap surgery

I arrive at the hospital which is an hour drive away for my anal advancement flap surgery. Met with the doctor and talked about what he will be doing depending on what he finds. Sounds like he found a pretty nasty scar band in me, raised and hard so he cut it out and replaced it with tissue from outside the bum, it sounds like my fissure was also close in proximity so he was able to cover that up as well. The doctor seemed to be very confident that he fixed me. The proof is in the pudding as they say so only time will tell. Left the hospital with a prescription for oxycodone, but he told me if I need anything just to call and he will give me anything that I need higher dosage, muscle relaxers etc so that's nice. I went all day yesterday without any pain, only some mild discomfort when getting in or out of bed or bending over. This is nice because of after the large amount of pain I was in from the hemorrhoidectomy and spinctorotomy I thought for sure I would be be in a lot of pain but I'm not yayy. With that said I woke up at 630am not exactly in pain but uncomfortable enough to not be able to continue sleeping. Normally I have my bowel movements around 730 in the morning it is now 9:06am and still no BM. I'm gassy and I suspect it won't be too much longer until I go to pass it. This will be the real test on how pain management will be during this recovery process. Stay tuned for the next update

I sympathize with you about what you are going through .I am also going through a long puzzle of surgeries which it is also extremely painful. I went for a checkup with my CRS yesterday and he was really thorough,but it now hurts like HELL! Next time I will speak up and let him know not to be so heavy handed . Please keep us updated Jose . Good luck with your recovery

Thank you for posting your very detailed experience. I'm sorry that you have suffered so much, but sharing the story with others will be very helpful to those who follow. Welcome to the forum, and I hope that things improve for you soon.

Thanks for the reply, I am sure most people on this site has felt just as much pain as I have been going through whether or not they have been through the same procedures. In a perfect world I would have been fixed after the first surgery and I never would have got to this point, but unfortunately it didn't work that way for me. It's a very sensitive area and being a person who has a BM every single day it's a crappy situation to be in. Thanks for the link I'll go check it out

Day 1 continued
It is now 4:34 pm I had my First BM since the anal flap surgery a little before 4. The BM was nice and soft and took just about no effort on my end to pass which is great. I did pass a piece of gauze but I'm not worried about it I assume it was just a small packing they did to collect blood. So the passing was painless. I hovered over the toilet instead of actually sitting on it. Showered immediately afterwards with warm water and then used my wife's hair dryer (Cool setting) to dry the area. I started to feel some tightness and a slight burning sensation after all of that. It's been about 40 minutes and the slight pain is still lingering but it's not horrible compared to the pain I had before the surgery. No spasms so I'm thinking the burning and slight pain is due to the stool sliding across the surgical area and causing irritation. Hopefully in an hour it will all calm down or maybe with another warm soak. In a nut shell day one seems like a great success no pain before the BM and minimal pain after the BM. O and my butt is still swollen but I felt down there with my hand and it seems like my stitches come out of my butt about an inch. A lot further then I expected, but then again there was a lot of damage which needed fixing so fingers crossed staying positive and hoping day 2 is just as easy.

Day 2
Not a whole lot to post. Had a soft BM but it took 3 trips to get everything out, before the pain could start calming down. Pain still isn't horrible but slightly higher then day 1. I was worried maybe I took to many oxycodone and didn't drink enough water. So I only took 1 oxycodone on day 2.
Day 3
Another soft bowel movement, no pain after just itchiness which I assume is a combination of the wound healing and stool sliding across the stitches. About 40 mins later I had to go to the bathroom again but it was only a tiny bit but it caused me pain and I have been very uncomfortable since then. I'm not sure what I'll end up tweaking to help out tomorrow. I've only taken one pain Paul today and I might take another cause the pain doesn't seem to want to go away. I'm super worried about ripping my stitches so I don't know if it's me worrying about that, that is keeping me from having a single BM or if it's something else. It almost feels like a pressure issue. Like I have enough to get out the bigger stuff, but not enough to expel the smaller tail end. Then that gets stuck at the surgical site causing the discomfort and pain. Between the fiber and stool softner I'm normally full of gas, which requires me to let out some extraordinarily long and loud toots throughout the day just to keep the pressure down. I want to try a mild laxative, but I'm afraid of what that will do. Might make my stool to watery, it might make me push to hard while having a BM. I guess I can still say so far in better then before the surgery, but it seems like each passing day the pain slightly increases. Still on my pudding and jello routine, I really want to eat some real food(maybe I will take my chances and have a salad for dinner tonight.
