Ok, Lars. Does the bowel movement seem to set it off? Or does it seem to not matter about the BM?
Man, I know what you must be going through... and no real help from your CRS!

I just wonder why it would burn all day. I have always associated the burning with getting wet fecal matter in the wound, like having a loose BM. That was my pattern.
I did, once, have an all-day episode of spasming that came and went and I recall there was a burning-type pain associated with that particular episode. Is your's like that, does it come and go?

I developed whole day burning with Nitro first time around. It bothered me so much that I stopped using it for coupe of days but than my fissure started acting up again : ( !
Now - second time around I alternate Nitro with Nifedapamine during the day and burning is less, but whenever I apply Nitro burning starts immediately and lasts for couple of hours.
I think that it is the result of capillaries dilating too much and causing pruritus ani - which is medical term for "burning or itching butt".
Some other members , like Val, developed extreme sensitivity to the creams and had to stop using them, so , yeah, I think your burning could as well be a reaction to the creams.
Best regards !

I had more burning than I did spasming with my fissure. It felt like an open wound which is exactly what it was. I took lots of sitzbaths and took 800 mg of ibuprofen 3 times a day for awhile there because it hurt so badly.

I am sorry you are having a rough go of things.

I still have occasional burning with the nitro cream as well - even after using it for months! I has got better - when I first started using it, it was alot worse. :)

I don`t use any creams so it isn`t that who is making it burn...Last friday I had botox. Maybe it is just spasm pain who feels like burning/sharp pain. If it don`t burn I just have soreness or itching...Have anyone used xylocian gel 2% with success? I will get my doctor to subscribe some of that to me tomorrow.

My nitro cream has lidocaine (same as xylocain) in it. Sometimes things can feel a bit numb down there - but a side effect of xylocaine is burning! Go figure! I think it is still worth a try!

Sorry Lars, I forgot about your Botox ! I think you should go to your CRS and make him check the area since injections can sometimes irritate hemorrhoids even if you never have them before. It is always better to check things - if nothing just to have a peace of mind and stop worrying : ( !
Jen - what was the mentholated cream that you used to calm tags? What do you think - could that help mask the burning for Lars?
