Hey Pgre,
I think with a big BM, you are tensing up your sphincter and there isn't as much option for relaxing as you need to go.
But with dilating, there is no urgency (unless your 9 year old daughter is banging on the bathroom door!) and you go at your own pace. You can feel when you are stretching too much and back off.
Like TP says, slow and cautious!

Well,
I moved up to the dilator that is a wee bit bigger than an inch... like 1.06"
It looks so big, but that's what it is. Just an inch.
Feel fine afterwards. In fact I feel better than I did earlier this morning or yesterday.
I am discovering though, that the water based lubes, even the sliquid I got, just aren't slick enough for the larger dilators. So I am just using these two olive oil/wax based lubes I have. One is a nipple cream for breastfeeding moms called motherlove. I used it for the first time today as it has calendula in it which is supposed to be pretty good for healing cuts.
So far so good. Frustratingly slow, this healing business.

for cleanup, I just use a non-scented shower gel I got from whole foods (or as my hubby calls it, 'whole paycheque') :)

I've found the same thing with water based lubes - olive oil is the best.

I finally was able to use the .8 inch dilator with ky jelly !! It hurt slightly and I could feel my fissure stretch.. no blood or spasm so far. Keeping my finger crossed and hope no pain during .bm. Tomorrow.. TP

Baba,
I am finding the 1" tough going. I think I will try plain old Olive oil tomorrow. I was reading that it has a lot of natural vitamin E in it!
I may just need to find a different shaped dilator though. Something not as blunt on the end as the vaginal ones I have.

you are right Tanya, I do find nd the vaginal dilator hard to insert. I did get the ease in dilator, the shape is good but I cannot stand the plastic smell. The next best thing would be the dilaton ones. But the smallest set I got had a deffect, even after I exchanged it. Tp

I just ordered the kind Baba has (colt trainers). I was looking at the dilatan ones too, but they are all in the US.
Ah well, one day at a time eh : )
I never did see that TCM doc. I felt like if I am already healing, why rock the boat. But I was kinda scared anything they did might make me worse. I still think about it, as a fall back option. I've given up on western docs in this area. The one think I liked about the TCM clinic is their slogan on their website says, 'we care'. SO simple but something that has been lacking in the western clinics. They couldn't have cared less!
Well it's a lovely day here, and I should get my tush outdoors! soak up some vitamin D.

Tanya, just take it very slowly. The butt plugs are very tapered so probably they would be easier to use than vaginal ones. Sometimes it works better to get things relaxed with your finger before using the dilator.
I'm using KY jelly now instead of oil because it was pointed out to me, and it says on my dilator box that you should use only water based lubricant. Too bad, because olive oil works very much better. Yet I had been using olive oil and the dilators don't look any the worse for it.

Baba, thanks, yes I guess I'll switch then too.
I wasn't crazy about straight up olive oil either. So messy and seemed to soak up faster than the kinds I use that are mixed with a bit of wax and stuff.
I have gone back to the 0.8" as the larger was just too difficult. I can feel the muscles that are stretched and they kind of radiate out from the front of my sphincter. I looked them up and they are called the Transversus Perinei.
i've always wondered if my AF didn't have something to do with the episiotomy I had for my daughter's forceps delivery. That these muscles are so tight and inflexible seems to lend credence to that theory.

SO best to take it slow, and the smaller size dilator seems to help anyways. Perhaps I don't need to go bigger?
I think of it like when I do the stretch where you sit with your legs out before you and try to touch your toes. If I do this one every day, after 2-3 months I can get my head down to my knees. So perhaps after 2-3 months of dilating, I will be able to get those muscles to loosen up.

Here is what a doctor told me about the child birth and fissure. During child birth the muscles strech and end up with small tears. When these small tears heal muscles loose some elasticity. When there is big BM, mucles cannot strech enough causing tears, which causes even more scar tissue and eventually spasms. Which makes sense why so many new moms end up with Fissure.
I am still at .5 inch dilator, .8 seems too tough right now. I do see difference even with the finger massage and the .5 inch dialtor !
