Volans, the ointment used to relax the internal sphincter will not work on the pelvic floor muscles. The ointment only works on what is smooth muscle (like in the digestive system) not on skeletal muscle (those we have control of). Valium should work on the skeletal muscles...maybe you could find a Doc that would prescribe it as an ointment?
I know what you mean about not tolerating anything up there. Sadly, for me it took botox to get me in the door (the back door that is). Now I have to wait to heal these fissures first in order to resume what I was doing....it's been almost a year with this fissure problem and feel like I have lost all my gains

Dec '13 Fissure from anoscope
3 X internal sphincter botox
'08-'15 Botox for pelvic floor dysfunction
Nov '14 LIS/sentinel tag removal
Feb '15 Deroofing of recurrent infection from LIS
summer '15-healed but still ongoing muscle dysfunction/pain
