Lost my post again!! grr!

Anyway thought I would post another update as this is exactly 2 months post botox.

I am pleased to report on the most part over the past 3 days I have been pain free!! it's an amazing thing to feel and I'm really over the moon. BM's don't hurt, it's just a grit feeling (a 0.5 on the scale if anything) and little to no resting pain throughout the day (again a 0.5 if anything!) No blood on BMs or on TP and it's just wonderful.

Before my fissure started 2 years ago I used to have a bm every 4 DAYS!! I remember the stool used to be huge and never caused me any issue (working on a ward ment I wasn't eating, drinking or having breaks so pooping went out the window) however since the AF happened I've changed everything dramatically. I now work 9-5 in a telephone triage service as a nurse and I'm drinking 2 litres of water a day, magnesium supplements every day, sitz baths 2 times daily with coconut oil and eating really good quality natural food. After botox I'm now going to the loo twice a day which was hell with the pain but is now a lot easier.

I still have the little fleshy lump just inside my sphincter at the 12 aclock position, it's sensetive to the touch but nothing really to shout about. Maybe it will go away with time?

Thank you all so much for all your support over the past year I'm keeping everything crossed this is it for me and I'm so so close to healing I can see the light at the end of the tunnel. I may be on here in a few days time crying sat on a hot bottle like before but I really hope not!!!! I'm not 100% yet but I'm feeling at least 95% there.

Big hugs to all and will keep you posted. On a side note I got a letter from southmead yesterday saying I've got my biofeedback and pressure test appointment booked for august!!! random! Didn't even know I was listed for one.

back to feeling around 80% which is still good but hey ho that's how things go! BM's still pain free but resting pain throughout the day is at around a 4/10 constantly.

First spasm in ages. It's not terrible but it's there post bm! No blood which I'm glad about. I feel like singing that song 'life is a roller coaster'

Weird place right now, I'm feeling 90% healed but this morning I had a bit of tugging pain down there so I had a look for the first time in ages and pulling back on the skin at 6pm I can clearly still see bright red, moist tissue that looks really sore as it was before my op. no change! the only difference is the level of pain I'm in so I'm guessing the fissure is still there it's the spasms that were causing me pain throughout the day that have now stopped because of the botox?!

secondly that lump at 12 I had a better look at today, looks like an internal hemi that he didn't band. something else to mention when I see him again in 3 weeks.

Sorry to hear about your discovery today Ff. Yes, the spasms do cause a lot of pain and the botox is very good at toning that down. There is also evidence that botox works on something called Substance P (a pain mediator) and can reduce pain in that way too. The thing with injuries down there is that you will feel better before it is actually healed. That is why it is so important to stick to one's healing program (ie., baths and stool softeners) for a very long time, ever after you think you are healed. Don't get down. There are likely aspects of healing going on down there that are not easily visible and some of your pain reduction must be that it is healing, it's just going to take a long time. DId your CRS ever tell you how long/wide and deep your fissure was?

never knew about Substance P (thanks for educating! always learning on here) I'm dreading when it wears off - I'm guessing the pain will be back with the spasms! I'm still having at least 1 sitz bath every day and using sudocrem in the eve. Still taking my stool softeners and magnesium which keep me regular.

It's so strange as my pain has been markedly increased today after looking down there and seeing the red/inflamed tissue. So easy to let it get in your head!!! I thought I was doing well but your so right, I'm most likely only starting to heal at the 2 month post botox mark and as always got to give it time.

only question is... do I go in for another round of botox? he wants to know in 3 weeks.

Well, ideally you wouldn't let the botox wear off and have those spasms return. My CRS said it lasts 3 months in the internal sphincter. Are you still using fissure ointment? It would likely help with making the botox last longer (it did for me). I still use Nifedipine after my LIS.

I know what you mean about how the psyche plays a role in pain perception. I STILL have not looked down there since my LIS over 7 months ago. I just can't as I know I would likely 'feel' more pain. Luckily my dear husband has been my eyes through all this. Of course your pain may also be increased by pulling at the skin as well. I am so delicate down there that would flare me up. I try to touch the area as little as possible.

Well, what did you think of the botox? If it is working well enough then why not try another round? Is your CRS open to LIS if you don't want to try the botox again?

i'm up for more botox, I had no side effects other than a rush to the loo every day!
My crs really doesn't want to do LIS on me but said it was up to me ultimately at the end of the day.

maybe I should opt for botox then LIS if that doesnt help?

That is what I did. I think it would have worked too if I didn't also have pelvic floor dysfunction. That BM urgency made my external sphincter and pelvic floor muscles go crazy, making my BMs damaging! There were times on the botox that I really thought I was going to heal but then it would wear off so my advice would be just to make sure it doesn't wear off. You also don't want too much though as then that urgency becomes worse and maybe even incontinence can happen. It's very difficult to get the right dose.
