Perhaps the reason you couldn't pass gas yesterday was because you did have some sort of packing up inside?
I had pain for the first few days, but it wasn't severe. I didn't know, however, if the pain originated at the incision site, or the fissure, or was from one of the new skin tags that were generated by the surgery. One of those tags was very, very sore, but only if touched by (even) the saturated toiled paper that I was using to dab myself carefully. It was excruciating, even if a tiny corner of the paper touched it. When the whole area is traumatized, as it is with surgery, it can be impossible to determine what exactly the source is, because pain radiates to the whole surrounding area.
Are you sure that they did a LIS? I can't understand why there would be stitches going into your anus since the cut was to the internal sphincter. I have only heard of them cutting to the side and inserting an instrument to go up inside to cut the internal sphincter, or going up into the hole and cutting it up there. Neither would produce a cut that would include the lower sphincter at all. Also, the nurse shouldn't have expected you to have a lot of pain for a week, unless a different procedure was done, or that the first BM would be really painful. Both of those expectations seem more in line with a hemorrhoidectomy or a fissurectomy (IMO).
When you say that the nurse said that 5mg of morphine, "sounds about right", did you also ask if you could safely take more?
Before my LIS, I was offered the choice of doing an enema on the morning of the procedure before I went in, or having them administer the enema once I arrived at the hospital. I chose to do it myself, but maybe they just gave one to you after you were sedated because that is the standard method at your hospital (or with your surgeon).
I hope that things improve for you soon, and who knows, if you did have packing in there, maybe you'll be able to pass gas soon too.
