Hi everyone. well let me begin to interduce myself. My name is darren, 40 yr old male from scotland u.k :D . I have learning difficulties after having an accident at work 3 yrs ago. My spelling is now very poor at time so excuse my use of english, grammar etc etc at times Image it very embarrasing Image
anyway lets get started to my fissure problems. all this began dec08, when i strained to hard and caused as was thought by my doctor an external hemmeriod i got proctosedyl Ointment for this. over the next week or so this developed in to a slightly large lump swelling up my perianal tract, i was in immense pain, still the doctor just said use the cream, it wil go,
anway it did go about after 2 weeks of sheer agony and many a day in my bed Image .
anyway this left a lump in my perianal area just under the skin. just heading towards my front end. i call it 6 oclock area. i put up with this until 2 weeks ago when afer a B.M I felt it swell again, and tere was blood in the toilet bowl, lots wen i wipe myself, wipe myself 2 hrs later and still blood on te tissues, i got up the following day still blood, so i went to my local a &e who said this was a perianal tear, which away bak in dec08 caused an perianal abcess Image Image they gave me very strong anti-biotics for seven days, the lump still came and went after a B.M.
just wednesday the 21st of jan 09 i was admitted to hospital. i was put on drips etc, saw 5 different doctor who all agreed the lump need removed and i needed a colonascope. as i had fissure inside going right to inside. i was fasted for 24 hrs nil by mouth, no water even, everything i need was put through drips, i was booked in for the op yesterday morn at 9 am.
8.30 the specialist comes along for the 1st time has a loook, nd say no op... Image he put me on diltiazem(anoheal). so i got this cream and sent home, i have used it around 5 times since yesterday had no blood today after B.M, but when i got up this morning there was like leakage on y underwear, not blood, not poo but maybe a mixture, am used 2 seening blood every morning on my underwear, but this was different today. today i feel as if i am leaking at times, wen i clean myself there is blood/mucus/ what every it is on my tissue,
can diltiazem(anoheal) make this happen.... and also can i use diltiazem(anoheal) inside my anus, or is it just for external fissures. the instructions are a bit mixed to me. the say se about 1inch lenght of cream in and arounf the anus... but then goes on to say for exteral use only.... caa any1 help, and answer some of my questions.
questions
1. can a fissure bleed for 12 hrs plus from heavy to very little
2,diltiazem(anoheal) can this cause anal leakage.
3, can i use diltiazem(anoheal), inside my anal passage
4, any1 else got any idea`s tips for me to try,

Welcome Darren! There has been confusion before about the "for external use" only label on the diltiazem. I used nefedipine which is the same class of drug and I applied it up fairly high into my anus using an applicator and I got my best results this way. Anoheal is a blood pressure drug, the attempt is to lover the tightness in your rectum so the blood vessels there won't be constricted and they can heal the fissure. Being too tight down there = no healing....
So my reasoning was get the cream up there to loosen things as much as possible. i never had any bad effects from applying it higher than just the outside of the anus.
Fissure can bleed as you describe but that's a lot of bleeding. Remember, though, a little blood can look like a lot!
I have heard of people being sensitive to diltiazem but not leakage. That is only my anecdotal knowledge.
Yes, I used it inside the anal canal and unless you are sensitive to it, you should be able to, also.
As for tips, this board is full of them. Read the past posts and learn, but I will remind you that frequent hot soaking baths are very good for increasing blood flow, and you should take as many as you can tolerate and have time for every day!

Darren,
First of all welcome! I used diltiazem ointment for several months and still use it occasionally when needed. My Mayo CRS advised me to put it inside 1 cm. I think external means just that--not to be ingested by mouth. You are not alone here; in fact there is another person on this board from Scotland. You will find a lot of support and answers to many of your questions from others who have faced similar situations. Good luck!! Cheryl

thank you kim for ur very detailed reply
i have 3 baths per, and always use unscented baby wipes after i wipe my self with tissues, the hospital specalist told me to use diltiazem cream 4 times aday,
also one thing i forgot to mention was do fisures get more sore, walking, bending, siting down etc,
yes wen i finished posting my prob, i went and had a look around and notice lots of different topics.. i will be having a look very close tonight,

All of the above, walking, stretching, kneeling, bending, can all irritate/aggravate your fissure.
While I was healing from my LIS, I laid around a lot because I had time off from work. On days when things are at their worst, you should do all you can to just lie around, read, watch TV until things feel better. Of course, we all can't just lie around everyday, can we?

no not everyone can just st around even in pain, mothers, fathers, carers etc we all have got things needing done everyday, but i wish to god everyone with fissure pain could. :D

I am sorry to read your story sounds like the typical NHS style !!
I have used the anoheal cream for a very long time so I consider myself an expert :roll:
For sure you need to get that cream high up – use an applicator like the ones that come with anusol, massage some cream on first and bobs your uncle the applicator should glide in !
In my own experience with fissure I only had the blood when having bowel movements, I would be a little concerned if I was still bleeding hours later you
mention mucus and such too - the cream can loosen one up but I don’t think it can cause anal leakage like that.
Fissures can be soo painful walking, standing, sitting can be very uncomfortable Image
Has the pain increased since having the leakage ???

First of all WELCOME Derryboy- sorry you have to be here, fissure pain is evil.
You seem to be doing all the right things to get healed. You say you had an abcess? Did it totally heal up? That could account for the leakage. I used diltiazem just on the outside and it still worked, but I was squeamish so thats why I couldnt put it in further!
The diltiazem is good stuff but takes at least a week or two to work. I think if you still have major leakage and it looks like an infection maybe you should go back to your doctor.

Tabby Image for ur very good advice, no the pain has not increased as yet, the leakage just happens every now and then, so when i go for a wee i always wipe then baby wipe my self clean, but there is always something to wipe away, its not go a smell or anything, its is though an added worry. one of the doctors at 9 wells did say when he had a look inside just by opening my anal up slightly said it look and very inflamed inside, so he went no further with the digit exmananation. but no more was said.... so am clue less.. last night a i had softish B.M. with very slight blood, wen i wiped, this morning i had diahorrea, and again slighty more blood, than last night, by around 4 oclock this afternoon i felt a slight leak happening, it stopped but as i type it fel as if its happening again :( but the pain is no worse......
val ..... i to am very ssqueamich, but i was able to put proctosedyl Ointment in there so i will try with anoheal. but wot worres me is if this is an infection in there, how will it react to the anoheal, cudnt stand more pain Image
