I know Darren - its so not the type of thing we want to discuss Image
I hate the thought of your pain :(
Maybe the next plan of action is to ask for a referral to the colorectal surgeon i mentioned it can take a few weeks, i would insist to your GP that this pain you are having is unbearable and you would prefer to have a specialist opinion

What do you eat Darren? I turn to liquid food (smoothies) when things are bad and it helps. I also use lactulose and Movicol, olive oil, fish oil...no coffeine, no alcohol, etc.
Do you keep the are dry and clean?
Dreading the next pooh is a nightmare Image you need to see a CRS asap!

I was taking 15mls lactulose 2x per day, this was upped to 30mls 2x aday. but this all ended in giving me a very rubble tummy and very gassy butt lol.
not a big fan of coffe or tea, so its water, 2/3 bottles per day, hardly any alcohol, maybe around 35 units per month.
i eat all 2 bits of toast in the moring, then pasta dishes, or fruit salads at lunch, and fish/chicken/mince/ beef dishes at dinner, nothing after 7, apart from orange /apple juice,
after every B.M i use toilet wipes, unsented, and now have started the hairdrying thing since sat/sun.
i also not forgetting like sardines/salmon, in sunflower oil,
i was also using vasiline b4 i started gettn cream,
i know this may look like i eat perfect, but i do at times love chips and fish, all the bad things but not alot,
am just at a loss,

Does your poop come out pretty easily? Does it feel like some debris gets stuck in the fissure or does it feel scratchy when it comes out?
I couldn't take the lactulose either for the same reason. They don't call me Tootie for nothin'. Movicol should help. Also a high fiber cereal really helps me called All Bran Buds by Kelloggs. I don't know if they have it over there or not. But I bet there is something similar.
How many bms do you have a day?

All bran buds and flax seed oil (not together) really help me. If I take one spoonful of flaxseed oil in the am and one in the pm it really helps the bm slip out better. The all bran buds give it a bit of momentum.

Wouldn't it be better to shower after BM, and take sitz baths?
Have you tried eating very little and only liquid food?
What hurts most, the beginning or the end of the BM?
Alcohol is very dehydrating...

Eat fiber (fruits and veggies and whole grains) and cut back on the beef. Only eat enough meat to supply adequate protein as meat, especially beef, will make for lumpy, mis-shapen stools that will grind your fissure as they pass by. Use a stool softener. You really don't need to drink tons of water, just get a normal amount, you're just gonna pee away the excess. If you don;t get enough fiber in your diet, try metamucil or benefiber, but too much fiber will make for extra large stools so watch out!
When you have your ale, just make sure to also drink the same amount of water/fluid to replace it. Same with coffee.
Good luck, sorry you are in such pain, give the nefedipine a while to kick in..
