In terms of cleaning, here was my routine (based on the advice of my CRS):
I took at least three 20 to 30 minute hot baths a day.
After a BM, I did NOT wipe at all. I hopped in the shower and gently rinsed off with a hand-held shower head (without any soap). I then hopped in the bath and let everything soak for 20 to 30 minutes (again, I didn't use any soap). After the bath, I hopped back in the shower and this time I used a very mild soap (Cetaphil) to clean the area gently and thoroughly. After the shower, I used a hair dryer (on the cool setting) to dry everything off. This kept everything nice and clean.
In terms of narcotics, I am not a doctor, nurse or pharmacist, but this is my understanding based on what my CRS told me: one of the most consistent side effects of narcotic use is constipation (which is why many CRS's do not prescribe them following surgery - the last thing you want to do is strain to have a BM). To combat this, you might want to take Milk of Magnesia or Sennakot, following the directions on the bottle. Both are available over-the-counter. Of course, I would check with your doctor if you are experiencing any constipation before taking any medication. Keep in mind that it is normal for it to take several days to have a BM following surgery. If your first BM is anything like mine, it will be a breeze (the fear of the first BM is much worse than the actual BM, in my experience).
