Yup, diltiazem in the morning and nitro at night.
- Fissure developed in Jan '13
- Started rectogesic in Feb '13 and diltiazem Apr '13.
- Got botox Jun '13
- Healed by Okt '13, although I still had some irritation for about a year.
- New fissure April '22, healed June 2022
- New fissure 24. December 2023

BTW. when applying diltiazem today, I noticed Im still tight as a drum down there. I think the progress I had was due to cowpad BMs. Dont think the botox have any effect, as I still have spasms like before, and still feel just as tight.

Do you feel the same tight during the day before BM?
I heard botox take time to kick in, 2-3 weeks. Probably you are 3 weeks.
How much units of botox you had? How many injections?

Im alway most tight after BM. Then it loosens up during the day.
I have no idea how much I got, or how many shots. Guess I will just have to wait and see if and when it kicks in. Untill then I will work on getting my BM back to bristol 5-6...

Hi JHH,
If you have IBS issues, it is very hard to control BM consistency. I know this too well myself... I have had made excellent progress on the FODMAP plan.This is a diet devised by the Monash University Australia.It has had a 75% success rate with IBS sufferers. I feel great after 2 months on the plan. I am Gluten and Wheat free, Lactose Free and eating the recommended Fruit and Veg types.I am having much better BM'S and the symptoms have decreased immensely.Have you been tested for any bowel/stomach disorders? Couldn't remember if you had?! Maybe you could ask to see a Dietician if so, as Diet may be the key to handling and controlling, and of course healing the damn Fissure. Please don't give up hope, i agree with Igrik on the fact that the Botox may not have fully Kicked in. I knew when it had, as it felt a lot looser back there and i was not having any spasms or pain on BM, as i said this took 2-3 weeks for me.ImageImage

Thanks Rachael and igrik. Lots of good ideas in the replies here. 
At first my doctor thought it was a peptic ulcer, and I was on acid inhibiting medicine, then penicillin. Later a gastroscooy showed no ulcer, so now I dont know. I have always had loose BMs since my teenager years, and I still never gets constipated. 
I just find it strange my CRS says bristol 4 is ideal for a fissure, when 6 works best for me...

Day 13
Yet another day with formed BM, although it seemed a bit softer today. Maybe Im getting back to bristol 5-6 soon :)
Had pain for some hours, but not as bad as the other two days, and about 13.00 I was painfree.

Day 14
Just had the hardest BM yet. Looked like a bristol 3. Dont know what is happening, but Im unable to control my BMs at the moment :(
I only use psyllium husk and magnesium, since I cant tolerate movicol, and I have increased both during the past 3 days, with the opposite effect. Any ideas welcome, cause this will be a very painfull day!

Have you tried lactulose?
I find ground flax seeds from holland and Barrett added to soup and a good glug of olive oil a good aid to stool softening.
Also, before I was on movicol, I bought some 'natural calm'. It's a magnesium supplement.. It seemed to have a stool softening effect too. I bought that from amazon.
I hope you are not suffering after that bm for the rest of the day.

I agree with OMB, Try lactulose and the cold milled flax seeds twice a day have really helped soften up my BM, but keep it formed.If it is too soft and squigy it burns. I also take Magnesium twice a day, chelated magnesium tablets as these are absorbed by the body and help relax the muscles.
