Set back

Hours in bed: awake whole night, several hot baths
Pain next day: 1-2
Long wait between BMs doesn't seem to work anymore. Started with Bristol 5, then some 4, eventually 6. Soft enough in theory, but apparently too large and not very self-running. I tried not to strain, but I had to open my anus larger than usual and this seems to cause damage.
Taking decision to adjust strategy and go on stool softeners in order to achieve a BM at least once every other day. My body seemed to make a habit out of pooping rarely and now I don't even get the urge to go anymore.

So I think Botox has now officially worn off, 2 months past injection.
Damn it, I didn't know spinach constipates. It looked so green and mushy, totally fissure friendly. But it gave me a piece of Bristol 3.5-4 that did the damage. The rest was 6, very delicate on the fissure, but what's done is done.
Stayed way to much in the bath tub and water way too hot, up to 42 C. So by the morning the inflammation turned into a thrombosed hem, very painful, hard and blue. Ointment and medication calmed it down.
Next day I feel totally torn.
How did this happen? I was at zero pain exactly 4 weeks ago when the fissure was larger and more raw. Why does is hurt so much then?

I have been reading your diary. Your recent posts depicts that you are in pain. You have tried botox that seems not very much helpful for you. What other options has your crs discussed with you?

Bgd. Sorry that things aren't going so well. From reading your diary, these are my few thoughts.
You do not mention whether you actually had the cauterization. Did you have that done as you said your doctor spoke of the danger of a fistula if not?
Much better to go every day, especially as you say you seem to have lost the urge sensation. Do you take any type of stool softener or something like Milk of Magnesia? I take both, though quite small doses.
I would not eat meat. I have become a total vegetarian and I also eat small meals, in my case only one meal a day. And go for only about 25g of fibre a day.
Awful that because of the pain you have difficulty sleeping through the night. Have you spoken to your doctor about
that? Very debilitating not to sleep right.
I see that you are in Romania. Do they perform anal dilations there and has your doctor suggested this? Can help the fissure heal. wecc - a member - had this done with great success.
Really feel for you. Fissures are awful!

bgd, I think perhaps now that the botox is worn out you need to have a serious talk with your doctor to see what is next, after all you have been suffering a lot.
I hope things get better.

Better days. OK, so I'm now pretty sure the pain last time was from the thrombosed hem and not from the fissure. I learned to distinguish between the two types of pain. No thrombose this time, thank to the medication prescribed by the doctor.

[color=#000000][color=#000000]I was stupid enough to stop the thrombose treatment and it came back, luckily milder.[/color][/color]
[color=#000000][color=#000000]For the first time I took an organic stool softener and it made me go 2 days after the previous BM. Stool was also a bit softer and it didn't iritate, sure sign that the fissure is not that raw anymore. I'll stick to this formula.[/color][/color]

Thanks all for the thoughts!
After a short set back I managed to get myself back on track. It is actually a second problem, my fissure seems to be fine. My hems were passive so far but for some reason one of the external ones started to thrombose.
Going to the CRS on Thursday. To answer your question:
- didn't do the 2nd cauterization yet, kept avoiding my CRS :) perhaps on Thursday
- they only do manual dilation, my CRS doesn't recommend. But let's see how the fissure is doing upon check-up, my feeling is that it did some healing. Actually I might need to have the external hems banded first
- not much meat here either, something like 3-4 very small cubes every other day. Mostly vegetarian for the past couple of months
- 1 meal per day, plus fruits and vegetables morning/evening
I was contemplating a second Botox injection, but now that I identified that the pain is from the hem, I might not need it. Right now I feel the sphincter quite relaxed. Let's see how the check up goes

Checkup June 21st

Checkup went well today, CRS said I achieved a great deal of healing. Fissure shrank to a triangle of 5x5mm. Started at 15mm 2.5 months ago, then less then 10mm a month ago. That's about 50% healing per month, not too bad. Pain during checkup was also insignificant, compared to last time.
Botox not needed anymore, CRS said. Although sometimes I feel it's still on or maybe my sphincter pressure just returned to normal.
I also had the 2mm fold in the fissure cauterized using ultrasound. Fold broke and bled during finger exam, before cauterization. CRS explained that it probably breaks a little at every BM as sphincter extends, so cauterization is a must. Feel no pain right now, remains to be seen on the next BM. Last time I had cauterization done it turned really bad about 5 days later, but back then I had a large area cauterized, 1.5cm in length. So I'm optimistic it can't be that bad this time.
So it seems the last episodes of pain were due to the hems after all. CRS wants to start banding on the next appointment 2 weeks from now. Unfortunately I'm afraid it'll be just another source of pain and soreness, thus stimulating spasm and fissure pain also. I wonder if I should wait with that for a while. It just seems a lot, especially shortly after cauterization, which may take more than 2 weeks to heal.
